CONDITIONS:
1.In the event that any ground -disturbing activities or other project activities related to this development
uncovers protected cultural material (e.g., bones, shell, antler, horn, or stone tools) the attached Inadvertent
Archaeological and Historic Resources Discovery Plan must be implemented.
2.PanGEO must be on site during construction to monitor slope stability conditions and confirm construction
activities do not adversely affect site stability.
3.A letter, prepared by'PanGEO, verifying construction of the wall is in compliance with recommendations of
the geotechnical report must he submitted no later than 30 days after the walls have been finalled.
4.An as -built survey of the retaining wall is required after the wall is constructed This will provide baseline
data for monitoring any long-term movement of the wall.
5.At the time that additional timber lagging and/or additional tiebacks are required, based on the
pre -determined height established on sheet SS40 of the submitted plans, please contact the Planning
Department to inform them of the construction work that is to occur and to obtain permit approval.

PanGEO was on site on November 5`h and 6`h to observe installation of soldier piles for the permanent slope
stabilization along the west end of the property above the steep slope. The following summarizes our geotechnical
observations:
Soldier Pile Shoring Wall:
PanGEO observed the installation of drilled soldier piles this week. The project plans identify a total of
13 piles designated as J01 — J13 on the Jonasson property. This slope remediation is part of a larger
project that provides remediation for 4 homes in the vicinity of this property. The subcontractor
responsible for installing the drilled soldier piles is Pearson Drilling, Inc. (PDI). The drilling was
performed with a track -mounted hydraulic drill rig that was built by PDI. A Grove RT745 rubber -tire
crane was utilized for picking piles and providing support to the drill rig. Earthwork was performed with
a CAT 312 EL excavator and a mini -track excavator. The owner's surveyor provided hubs and offsets
for pile locations. The drilling for soldier piles was advanced with a 24 -inch diameter (26 -inch actual)
open -flight auger. PDI verified plumb on the auger by repeatedly placing a level on two sides of the
Kelly bar on the drill rig. The soils encountered consisted of dense to very dense brown sand with
occasional gravel beds to silty sand. The subsurface conditions appeared to be consistent with the design
assumptions as indicated in our geotechnical report. All drilled holes observed to date remained dry and
no caving or sloughing occurred in any of the excavations. PanGEO visually observed and taped the
bottom surfaces of the excavations with a weighted tape.
The steel beams for all pile locations to date were W 14X43 and were each 60' 2" in length. Piles
delivered to the site had corrosion protection applied to approximately the upper 33 feet of each pile.
Lean mix concrete (CalPortland mix design 1420) was placed in the excavations to approximately the
existing ground surface.
The contractor and homeowner requested a modification to the plans that would allow soldier piles
installed on the Jonasson property to be raised up to a maximum of 1 foot above plan tip elevation to
provide a more level back yard. PanGEO contacted the structural engineer by phone (CT Engineering —
Robert Thompson) and the homeowner sent an email requesting the modification to the project plans.

FIELD REPORT
The structural engineer approved the change in pile height for the Jonasson property in an email to the
property owner. Any changes to pile design at the other 3 properties will require additional
requests/approvals from the engineer.
Based on our observations, all drilled soldier piles installed this week were constructed in general
accordance with the approved plans and specifications. A summary of the as -built details of the soldier
piles is shown in Table 1, below.

observations:

1. Actual bottom elevations are based on measurements taken by contractor from offset hubs on site. Some elevations are
higher than plan elevation based on engineer (CT Engineering) approval to raise piles on Jonasson property up to 1 foot
above plan elevation. Actual pile lengths measured in field are 60' 2".
Slope Stability:
The steep slope appears to be stable. No new tension cracks were observed and no visual evidence of pile
movement was observed. PanGEO reviewed initial survey data on the Jonasson house which indicates minimal
movement within the range of typical survey scatter. The surveyors have to run control to the house from the main
road every time they perform a survey and that will likely introduce minimal scatter in the results. PanGEO will
continue to monitor survey results for indications of movement. Based on our observations in the field and review
of the survey data, it is our opinion that the construction activities are not affecting the adjacent structures and that
the shoring is performing adequately.

Hi Becky,
Attached is a profile produced by Eastside Consultants, our surveyor, with colored lines that I inserted to show the
location of the soldier pile, with one foot of exposure and the very slight amount of fill behind it. My rough calculation is
that the fill is about 15 cy, salvaged from the drilling spoils. By lagging that upper one foot, any sloughing should be
stopped from occurring and the yard is evened out a little bit near the crest. As you can see from the emails below, Rob
Thompson of CT Engineering gave his approval to the one foot of exposure. The as-builts that Eastside Consultants will
produce will show this condition.

Rob,
Curtis Mathis thought that I should ask you if you saw a problem with our leaving our soldier piles one foot higher than
the design top elevation. Terry Trew of Pearson Drilling suggested doing that to more level out the downturn at the
northern crest. It would mean one foot of lagging at the top and some added backfill. If that still fits within your
comfortable factor of safety, then we will leave the tops higher. If it does not, then we will keep them at their design
top elevations. Pearson will drill to the design bottoms for now and we need to make a decision on the tops by late this
afternoon when concrete will be placed. Please let Curtis or myself know.
