This information is also helpful to determine if available avoidance, minimization or compensatory mitigation measures will address the probable significant impacts or if an environmental impact statement will be prepared to further analyze the proposal. Instructions for Applicants This environmental checklist asks you to describe some basic information about your proposal. Please answer each question accurately and carefully, to the best of your knowledge. You may need to consult with an agency specialist or private consultant for some questions. You may use “not applicable” or "does not apply" only when you can explain why it does not apply and not when the answer is unknown. You may also attach or incorporate by reference additional studies reports. Complete and accurate answers to these questions often avoid delays with the SEPA process as well as later in the decision-making process. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Instructions for Lead Agencies Please adjust the format of this template as needed. Additional information may be necessary to evaluate the existing environment, all interrelated aspects of the proposal and an analysis of adverse impacts. The checklist is considered the first but not necessarily the only source of information needed to make an adequate threshold determination. Once a threshold determination is made, the lead agency is responsible for the completeness and accuracy of the checklist and other supporting documents. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. The project is to subdivide the 19.86-acre property as a cluster subdivision of 20 single family residential lots with a public road and tracts for open space, critical areas, and drainage facilities. The project density is 1.0 du/ac, which is half of what the RS-15.0 zoning designation allows. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. The subject property consists of one parcel situated in the Northeast ¼ of Section 12, Township 21 North, Range 3 East, W.M. (King County Tax Parcel Number 1221039037). The site is located just south of the intersection of 22nd Avenue SW and SW 309th Court. B. ENVIRONMENTAL ELEMENTS 1. Earth a. General description of the site: (circle one): Flat, rolling , hilly, steep slopes , mountainous, other b. What is the steepest slope on the site (approximate percent slope)? The steepest slopes on the site exceed 40 percent in localized areas. c. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any agricultural land of long-term commercial significance and whether the proposal results in removing any of these soils. The land is not agricultural or of long-term commercial significance. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. As noted in the Geotechnical Engineering Report by GeoResources dated December 22, 2022 (Section 4.1): "Erosion was observed at the northern terminus of the ravine that begins south of the end of 22nd Avenue SW. This erosion appears related to uncontrolled discharging of stormwater runoff from the adjacent plat, significant yard waste dumping into the ravine, and the past installation of the stormwater bypass line that extends south along the west side of the ravine. The old culvert discharged above the ravine and based on information obtained from the City, caused the erosion that created the ravine. Erosion was observed on the east side of the ravine in the form of exposed soils. Uncontrolled yard waste along the north side of the property has inhibited vegetation growth. Erosion was observed along the margins of the ravine headwalls and a landslide deposit is mapped in the eastern portion of the site. "Since our original report was prepared, we understand that a landslide occurred about 700 feet north of the site on the slope that extends down from the top of the glacial upland area to SW Dash Point Road (SR 509). The landslide occurred on March 16, 2017. The City hired Landau Associates to do an initial review of the slide. In their July 6, 2017, Technical Memorandum to the City, they concluded that the rise of the “true” groundwater table within the deeper advanced outwash caused the shallow colluvium to fail, along with surface water from the ditch along SW 308th Street and above the failure. The topography, surface water runoff, and soil conditions differ in this area from the main Creekwood Plat property, and the area of the slide should have no impact on the proposed project." e. Describe the purpose, type, total area, and approximate quantities and total affected area of any filling, excavation, and grading proposed. Indicate source of fill. Minor cuts and fills will occur for site development of lots and the new road. New Road A, which crosses the ravine in the north-central portion of the site, will be constructed with an earth fill (using on-site soils) embankment supported by cast-in-place retaining wall. Approximately 37,000 cubic yards of grading will occur as balanced cut and fill with one consolidated site development activity for the clearing and grading of the new road and pad graded of lots. Of that grading, approximately 10,000 cubic yards will be topsoil strippings that will be placed back onto open spaces within the site, excess topsoil will be exported to an approved location. If needed, structural fill would come from an approved source. If needed, structural fill will come from an approved source. f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe. During construction erosion could occur as a result of precipitation events. During dry weather dust may also be generated during the earthwork phase. g. About what percent of the site will be covered with impervious surfaces after project construction (for example, asphalt or buildings)? Approximately 19 percent of the site will be covered by impervious surfaces, including Road A improvements along with future homes and driveways. h. Dust will be controlled by using a water truck as needed during the dry season. Completing construction of the roads and pad-graded lots will facilitate control of erosion and other construction impacts to one singular construction season. 2. Air a. What types of emissions to the air would result from the proposal during construction, operation, and maintenance when the project is completed? If any, generally describe and give approximate quantities if known. During construction, air emissions may include exhaust from construction equipment. After construction, air emissions may include vehicle exhaust, cooking smells, and wood- burning stove smoke typical of residential suburban development. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. Offsite sources of emissions include vehicle exhaust from nearby roads. c. Proposed measures to reduce or control emissions or other impacts to air, if any: No measures to reduce or control emissions are warranted or provided. 3. Water a. Surface Water: 1) Is there any surface water body on or in the immediate vicinity of the site (including year- round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. Yes, the site contains several streams and wetlands that contribute to Lakota Creek which flows to Puget Sound. As documented in their Wetland Report, Soundview Consultants “confirmed the presence and boundaries of the following potentially regulated critical areas onsite: three wetlands (Wetlands B, D, and E/F), three drainages (Drainages W, X, and Y), and one stream (Stream Z). In addition, one non-regulated seep (Seep C) was identified on the subject property, and two wetlands (Wetlands A and G) were identified offsite within 225 feet of the subject property. No other potentially regulated wetlands, waterbodies, or other fish and wildlife habitat were identified within 225 feet of the subject property during the site investigations”. 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. Yes, the project will work within 200 feet of on-site surface waters and wetlands. As documented in their Wetland Report, Soundview Consultants: “The Applicant proposes residential development of the subject property with a 20-lot residential plat and associated infrastructure including internal site access and parking, utilities, and stormwater infrastructure. The proposed project has been carefully designed to avoid impacts to the identified critical areas and associated buffers to the greatest extent feasible, and direct impacts, in-water work, and impacts to the buffers of Wetlands A, B, D, E/F, and G, Drainages W and X, and Stream Z are avoided entirely. However, due to the need to Department of Community Development 33325 8th Avenue South Federal Way, WA 98003-6325 253-835-2607; Fax 253-835-2609 www.cityoffederalway.com Bulletin #050 – October 17, 2016 Page 7 of 16 k:\Handouts\Environmental Checklist provide site access in compliance with the City of Federal Way’s street design standards, steep slopes which limit the developable areas onsite, the need to maintain site drainage patterns, and the location of Drainage Y near the northern boundary of the subject property, complete avoidance is not feasible. The proposed project requires necessary unavoidable impacts to 6,115 square feet of the buffer of Drainage Y to provide an access road across the northern boundary of the subject property that complies with the City of Federal Way’s street design standards and to provide a stormwater connection that maintains existing site drainage patterns following development of the subject property. The proposed work will result In approximately 148 square feet of permanent intrusion to the Drainage Y buffer due to the construction of a sidewalk and associated retaining wall. Temporary impacts into this buffer will extend beyond the permanent intrusion due to placement of fill and other grading activities associated with the road construction. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. No fill or dredging is proposed in surface waters. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. No surface water withdrawals or diversions are proposed. 5) Does the proposal lie within a 100-year floodplain? If so, note location on the site plan. The site is not located within a 100-year floodplain. 6) Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. The project is served by sanitary sewer service from Lakehaven Utility District which will convey all wastewater safely offsite in an underground sewer collection system. Stormwater runoff will be collected onsite in a closed pipe system and conveyed to an onsite storm detention vault and water quality system. The outfall from this system will connect to an existing 24-inch drainage pipe and catch basin system that discharges to Drainage Z on site. No new discharge location is proposed. The volume of stormwater discharge will be controlled to mimic existing flows as required by the 2021 KCSWDM. b. Ground Water: 1) Will groundwater be withdrawn from a well for drinking water or other purposes? If so, give a general description of the well, proposed uses and approximate quantities withdrawn from the well. Will water be discharged to groundwater? Give general description, purpose, and approximate quantities if known. No groundwater will be withdrawn for any purpose. 2) Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (for example: domestic sewage; industrial, containing the following chemicals. . .; agricultural; etc.). Describe the general size of the system, the number of such systems, the number of houses to be served (if applicable), or the number of animals or humans the system(s) are expected to serve. Water runoff (including stormwater): 1) Describe the source of runoff (including storm water) and method of collection and disposal, if any (include quantities, if known). Where will this water flow? Will this water flow into other waters? If so, describe. Runoff from the project will occur from precipitation falling on cleared and uncleared surfaces. Stormwater generated by the development will be collected, controlled and routed to an on-site detention vault and water quality facility located in Tract B before being discharged to an existing stormwater pipe in Tract C. Site drainage currently and will continue to flow to Lakota Creek and Puget Sound. 2) Could waste materials enter ground or surface waters? If so, generally describe. The water quality facility and erosion control measures as well as the sanitary sewer system will limit the risk of waste materials entering ground or surface waters through project generated stormwater. 3) Does the proposal alter or otherwise affect drainage patterns in the vicinity of the site? If so, describe. The proposal does not impact or affect drainage patterns in the vicinity of the site. d. Proposed measures to reduce or control surface, ground, and runoff water, and drainage pattern impacts, if any: All project-generated stormwater will be collected and conveyed to the on-site detention and water quality treatment facilities as noted above. Release of stormwater will be metered to mimic existing flows. 4. Plants a. Check the types of vegetation found on the site: X deciduous tree: alder, maple, aspen, other X evergreen tree: fir, cedar, pine, other hemlock X shrubs grass pasture crop or grain Orchards, vineyards or other permanent crops. X wet soil plants: cattail, buttercup, bullrush, skunk cabbage, other water plants: water lily, eelgrass, milfoil, other other types of vegetation b. What kind and amount of vegetation will be removed or altered? This will leave approximately 65 percent of the site undisturbed. c. List threatened and endangered species known to be on or near the site. The applicant is not aware of any threatened or endangered plant species on or near the site. d. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any: The project will preserve and protect vegetation on approximately 65 percent of the site. Mitigation and replacement are provided in open space areas where temporary impacts to vegetation will occur. e. List all noxious weeds and invasive species known to be on or near the site. Himalayan blackberry and English ivy have been observed on the site. 5. Animals a. List any birds and other animals which have been observed on or near the site or are known to be on or near the site. Examples include: birds: hawk, heron, eagle, songbirds, other: mammals: deer, bear, elk, beaver, other: racoon, rodents fish: bass, salmon, trout, herring, shellfish, other: b. List any threatened and endangered species known to be on or near the site. The applicant is not aware of any threatened or endangered animal species on or near the site. Stream Z is a tributary of Lakota Creek which is a salmonid stream. c. Is the site part of a migration route? If so, explain. The site and all of western Washington are within the Pacific Flyway of migratory birds. d. Proposed measures to preserve or enhance wildlife, if any: The project will preserve and protect vegetation on approximately 65 percent of the site. e. List any invasive animal species known to be on or near the site. The applicant is not aware of any invasive animal species on or near the site. 6. Energy and Natural Resources a. What kinds of energy (electric, natural gas, oil, wood stove, solar) will be used to meet the Department of Community Development 33325 8th Avenue South Federal Way, WA 98003-6325 253-835-2607; Fax 253-835-2609 www.cityoffederalway.com Bulletin #050 – October 17, 2016 Page 10 of 16 k:\Handouts\Environmental Checklist completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. The future homes will be served by electricity and natural gas for heating and energy needs. Wood stoves may be used for heating. b. Would your project affect the potential use of solar energy by adjacent properties? If so, generally describe. Use of solar energy by adjacent properties will not be impacted by the project. All future homes will comply with the city's height limitations. Removal of trees may increase neighboring property's ability to utilize solar energy. c. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts, if any: The future homes will comply with WESC energy codes and IRC for solar ready roof systems and electric vehicle charging-ready accommodations, if applicable. 7. Environmental Health a. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste, that could occur as a result of this proposal? If so, describe. 1) Describe any known or possible contamination at the site from present or past uses. There are no known past uses of the site; however, the site is within the Tacoma Smelter Plume (TSP) exposure area. As noted in the Limited Phase II Environmental Site Assessment performed by Earth Solutions NW, LLC (ESNW) some remediation will be necessary at the subject site in connection with elevated concentrations of arsenic in two composite forest duff locations associated with the positioning of the site in the TSP 2) Describe existing hazardous chemicals/conditions that might affect project development and design. This includes underground hazardous liquid and gas transmission pipelines located within the project area and in the vicinity. The site is within the Tacoma Smelter Plume (TSP) exposure area. 3) Describe any toxic or hazardous chemicals that might be stored, used, or produced during the project's development or construction, or at any time during the operating life of the project. No hazardous chemicals will be stored or produced by the project. 4) Describe special emergency services that might be required. Confirmation soil sampling will be completed at the subject site in redevelopment areas after soil mixing and/or within all stockpiles of diluted soil to verify that soil mixing activities and remediation of arsenic impacts were successful. b. Noise 1) What types of noise exist in the area which may affect your project (for example: traffic, equipment, operation, other)? Noise in the immediate vicinity includes traffic from adjacent streets. 2) What types and levels of noise would be created by or associated with the project on a short- term or a long-term basis (for example: traffic, construction, operation, other)? Indicate what hours noise would come from the site. During site construction noise from clearing and grading operations and equipment will occur. During home construction noise will occur from building construction, including saws, hammering, and music enjoyed by laborers. After construction is completed, noise from the site will be consistent with single family housing in a suburban setting. 3) Proposed measures to reduce or control noise impacts, if any: No noise control measures are warranted or proposed. 8. Land and Shoreline Use a. What is the current use of the site and adjacent properties? Will the proposal affect current land uses on nearby or adjacent properties? If so, describe. The site is currently vacant. Surrounding properties are single family to the north, commercial to the east, single family to the south, and vacant to the west. b. Has the project site been used as working farmlands or working forest lands? If so, describe. How much agricultural or forest land of long-term commercial significance will be converted to other uses as a result of the proposal, if any? If resource lands have not been designated, how many acres in farmland or forest land tax status will be converted to nonfarm or nonforest use? The site shows no evidence of prior use as a farm or forest land. 1) Will the proposal affect or be affected by surrounding working farm or forest land normal business operations, such as oversize equipment access, the application of pesticides, tilling, and harvesting? If so, how: The project will not impact or be affected by working farms or forests; this does not apply. c. Describe any structures on the site. There are no existing structures on the site. What is the current comprehensive plan designation of the site? The comprehensive plan designation for the site is low density residential. g. If applicable, what is the current shoreline master program designation of the site? The site is not within a shoreline; therefore, this does not apply. h. Has any part of the site been classified as a critical area by the city or county? If so, specify. Yes, the site contains several streams and wetlands (Wetlands A, B, D, E/F, and G, Drainages W and X, and Stream Z) as well as steep slopes. i. Approximately how many people would reside or work in the completed project? Approximately 50 – 60 people are expected to live in the 20 homes. j. Street and residential lighting will not cause a safety hazard or interfere with views. c. What existing off-site sources of light or glare may affect your proposal? Off-site lighting is from streetlights on 21st Avenue SW and security site lighting at the nearby commercial storage facility to the east. Neither light source will have any effect on the proposal. d. Proposed measures to reduce or control light and glare impacts, if any: All streetlights will be dark sky compliant and will include shut offs as needed to control light to the site. 12. Recreation a. What designated and informal recreational opportunities are in the immediate vicinity? Proposed measures to reduce or control impacts on recreation, including recreation opportunities to be provided by the project or applicant, if any: The applicant proposes to create a tract for the future implementation of a city-planned non-motorized multi-use path coincident with the sewer main easement in the southern portion of the site. Additional on-site recreation and open space areas are provided for the future residents to enjoy. 13. Historic and Cultural Preservation a. Are there any buildings, structures, or sites, located on or near the site that are over 45 years old listed in or eligible for listing in national, state, or local preservation registers? Please list any professional studies conducted at the site to identify such resources. The applicant is not aware of any landmarks, features, or other evidence of historic use or occupation. c. Each new home will have a minimum of two onsite parking spaces. d. The project will extend a short leg of 22nd Avenue SW to connect to a new cul-de-sac street that will connect to 21st Avenue SW at the site's east boundary. The new road will provide vehicular access to all 20 homes in the plat as well improve neighborhood connectivity by providing a second public road access into the plat of Lakota Ridge. e. Will the project or proposal use (or occur in the immediate vicinity of) water, rail, or air transportation? If so, generally describe. The site is not in the immediate vicinity of water, rail, or air transportation. f. How many vehicular trips per day would be generated by the completed project or proposal? If known, indicate when peak volumes would occur and what percentage of the volume would be trucks (such as commercial and nonpassenger vehicles). What data or transportation models were used to make these estimates? The 20-lot project is expected to generate approximately 189 ADT and 20 PM peak hour trips based on ITE Manual Version 10. g. Will the proposal interfere with, affect or be affected by the movement of agricultural and forest products on roads or streets in the area? If so, generally describe. The proposal is not in an area where agricultural or forest products are being transported; therefore, this does not apply. h.
