I can print out the information and submit it to the Permit Center, that's no problem. The only thing missing is a report/memo from a
qualified professional regarding the tree's health. Basically, do you have something in writing from your tree guy about the status of the
tree and what he plans on doing with it.Will the stump remain? Is the tree going to get taken away or chipped up, etc.

Following up on our phone conversation. It is my understand that you want to remove a dead/dying tree on your property(29232 13th PI
S).Your property is located within a Geologically Hazardous Area(GHA)erosion hazard area.Any tree removal is subject to review
under Federal Way Revise Code(FWRC)Chapter 19.145 Environmentally Critical Areas.
Per FWRC 19.145.120,the following activities are partial exemptions to the provisions of the chapter and require written approval from
the director:
(7)Vegetation maintenance such as hazard tree removal, removal of nuisance vegetation,and limited pruning for view
preservation.The applicant shall submit a vegetation maintenance plan prepared by a certified arborist or registered
landscape architect that includes the following:
(a)A site plan at appropriate scale denoting the extent of the proposed vegetation maintenance activity;
(b)Tree and vegetation location,type,and caliper of each tree within the area subject to the proposed vegetation
maintenance activity;
(c) Identification of methods of vegetation maintenance(limited to hand tools and hand powered tools);and
(d) Proposed tree and/or vegetation replacement shown on the site plan. (replacement not required for hazard tree on
residential property)
Please fill out the attached Request for Administrative Decision form and provide supporting documentation,such as an arborist report
evaluating the tree and photos of the tree—will the stump remain,tree remain on-site or be removed?Submit the form and documents
to the Permit Center,there is no fee for the Administrative Decision.
The City will review the documents and may contact you for a site visit if needed.A written response will be issued within 2-4 weeks. If
you have any additional questions please feel free to contact me.

The condition of the tree is Poor. The Risk Rating is: High.
Method:
I.S.A. Level 2—Ground level— visual observation - Tape Measure
— Laser Measure.
Findings:
The Subject tree is 50-inch Diameter at Breast Height (54") meas-
urement from ground, 45 feet tall. The Crown Spread Diameter/
Drip Line are 60 feet.

Crown and Branches:
There are large dominant top leads (8 to 4 then two at lowest union.) Approximately
75% Live Crown Ratio leaving 25% Necrotic. The Crown has been raised for clearance
creating a large amount of bio mass high from the ground. Decay at crotch unions make
this tree subject to Sudden Limb Fall especially during high wind events. There are visi-
ble signs of previous branch failures throughout the crown.
The canopy is very large (60' Diameter.) Increased wind influence makes the tree sub-
ject to failure in high rain, wind and or, snow load.

Trunk:
There are decay pockets and a large decay column at the base of this tree. The decay
column eminates from an old flush cut. (see picture below.)
Within this decay the is evidence of Ganoderma Mushroom growth which grows on de
caying sapwood. Also pictured below.

The decay and bark inclusion are creating a shear plane low in the trunk making this tree tree
prone to whole large lead failure.

The decay column is right along the shear plane. My opinion is: This is where the tree will fail
catastrophically.

Roots and Root Collar:
The tree is located on the top side of a down hill to Heavily trafficked Redondo Way South.
Failure would defiantly cause debris to fall, slide and roll to that thoughrofare. Also, Mr. Rob
ins Primary Residence structure as well as his neighboring structure and frequently occupied
back yards are all within reach when this tree fails.
Should this tree uproot, it would take away a large section of the edge of the top of the hill
leaving a place for water to gather at the top pf the slope.
