The proposal is to construct a new all-purpose synthetic turf field (approximately 30,000 square
feet) with lights and an appropriate drainage system located at the King County Boys and Girls
Club property in the city. The affected geographic area is approximately 4.22 acres spread
across three tax parcels.
12. Location of the proposal. Give sufficient information for a person to understand the
precise location of your proposed project, including a street address, if any, and section,
township, and range, if known. If a proposal would occur over a range of area, provide
the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map,
and topographic map, if reasonably available. While you should submit any plans
required by the agency, you are not required to duplicate maps or detailed plans
submitted with any permit applications related to this checklist.
The location of the proposal is at 30815 8th Avenue South, Federal Way, Washington 98003.
Township: 21 N, Range: 04E, Section: 08. The King County tax parcels affected by this
proposal are 0821049058, 0821049268, and 0821049119. A site plan, vicinity map, and site
grading (current and proposed) are provided in association with submission requirements of
the permit application.

Approximately 3,250 square yards of fine grading will occur on the location of the proposed field.
Approximately 610 cubic yards of excavation will occur for the creation of a pipe trench, which
will be filled with a compact gravel base and native backfill totaling the original 610 cubic yards.

Yes, erosion could occur during grading and the removal of vegetation in the affected
geographic area. However, the implementation of a temporary sediment and erosion control
plan using Best Management Practices should mitigate impacts.

construction (for example, asphalt or buildings)?
The total amount of impervious surface modeled for the affected geographic area is
approximately 8,900 square feet, consisting of 25% of the synthetic field area, an ADA parking
area, pavement patching, and walking paths.

• Clearing Limits — All areas to remain undisturbed during the construction of the project
will be delineated prior to any site clearing or grading.
• Cover Measures — Disturbed areas shall be covered as required in Section D.4.2 of the
KCSW DM.
• Construction Entrances — A stabilized construction entrance will be used during
construction of the project.
• Perimeter Protection - Filter fabric fences for ditch and site runoff protection will be
provided at the downstream site perimeter.
• Sediment Retention — Sediment traps will be used to treat runoff before discharge to the
new drainage/dispersal system during construction.
• Storm Drain Inlet Protection — Filter fabric protection will be provided on all new and
existing catch basins downstream of construction activities.
• Surface Water Control - Interceptor ditches will be used to direct runoff from construction
areas to the sediment traps and ponds.
Dust Control — Dust control measures will be implemented when exposed soils are dry
to the point that wind transport is possible and roadways, drainage ways, or surface
waters are likely to be impacted.
2. Air
a. What types of emissions to the air would result from the proposal during
construction, operation, and maintenance when the project is completed? If any,

Construction would result in a temporary increase in air pollution, including emissions from
equipment and dust from construction activities. With adherence to applicable construction
regulations, significant air quality impacts are not anticipated. Post -construction emissions
would include emissions from vehicle trips associated with the recreational use and air
emissions associated with equipment used for field maintenance.
b. Are there any off -site sources of emissions or odor that may affect your proposal? If
so, generally describe.
Vehicle traffic along 8th Avenue S is the primary source of off -site air emissions.
c. Proposed measures to reduce or control emissions or other impacts to air, if any:
Watering the ground as needed before and during clearing and grading activities will control
dust particles. Vehicles that are not being used in construction activities will be shut off.
3. Water
a. Surface Water:
1) Is there any surface water body on or in the immediate vicinity of the site (including
year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes,

This proposal will collect runoff and potential runoff and discharge it to two dispersal trenches
located to the north. The trenches will disperse the runoff to a native vegetated area that will be
protected as a Native Growth Protection Area. This area slopes to the north and will remain as -
is. Post Developed flows will be under 0.5 cfs and a dispersal trench will be utilized for the
outfall. Full Dispersion Trench Flow Control BMPs were sized according to KCSWDM Appendix
C.2.1.5 for the new synthetic turf field (10,000 sf of flow area per 35,000 sf nonnative pervious
surface). This 29,150 sf non-native pervious surface requires 8,835 sf of flowpath. Our 10LF
trench with 154 LF flowpath and the 44LF trench with 167 LF flowpath provide over 8,850 SF of
flowpath.

There is no anticipated waste material discharge created from this proposal. Any wastewater
created because of the proposal will be mitigated in accordance with the prepared stormwater
management protection plan.

The primary source of noise in the area is generated from vehicular traffic adjacent to the
proposal along 8th Avenue South.
Temporary, short-term noise impacts typical of construction activities will occur, consisting
primarily of equipment operation. Construction will normally occur between the hours of 7:00
a.m. and 6:00 p.m. Long-term noise impacts would be created by activities located on the
synthetic field and is consistent with other recreational uses. Off -site impacts from these
activities would be minimal. The hours this noise would occur varies based on scheduled
activities on the field.
C) Proposed measuresto reduce or control noise impacts, if any;
Noise impacts associated with the construction phase of the proposal will be limited in duration.
To mitigate general noise impacts during construction phases, measures such as using and
regularly maintaining efficient mufflers and quieting devices on all construction equipment and
vehicles will be taken. Construction hours will roughly be limited to the normal workday, 7:00
a.m. to 6:00 p.m. There are no specific recommendations to control noise impacts resulting
from recreational uses of the finished project.

illuminate the synthetic turf field. The proposal anticipates that the eight light poles will be thirty
feet in height.
Demolishing the existing vacant single-family residence and garage will alter the existing
perspective into the affected geographic area by removing all structures that previously blocked
views into the site. A six-foot chain -link fence along the eastern boundary of the proposal along
8th Avenue South will provide some visual opacity into the site.

mainly occur?
The proposal includes eight 30-foot light poles with twelve 1!500-watt lamps to illuminate the
fields. An illumination summary prepared for the proposal indicates that lightspill along the
eastern side of 81h Ave S will peak at 0.1horizontal footcandles and 0.6 vertical footcandles.
b. Could light or glare from the finished project be a safety hazard or interfere with
views?
Lighting produced by the proposal will be directed at the field. Shielded, and is unlikely to affect
views or be a safety hazard on or off -site.
c. What existing off -site sources of light or glare may affect your proposal?
No existing off -site sources of light affect the scope of the proposal

The Federal Way Historical Society, HistoryLink, and the University of Washington Library's
King County Snapshot websites were reviewed to assess the potential impacts to cultural and
historic resources on and near the affected geographic area.
d. Proposed measures to avoid, minimize, or compensate for loss, changes to, and
disturbance to resources. Please include plans for the above and any permits that
may be required.
If culturally significant objects are found during site preparation work, the Washington State
Office of Archaeology and Historic Preservation will be notified, and appropriate regulatory
measures will be taken.

describe proposed access to the existing street system. Show on site plans, if any.
Access to the proposal already exists along 8th Avenue S as a paved driveway that runs
adjacent to the proposed fields and provides access to the Boys and Girls Club parking lot.

stop?
The affected geographic area is not directly served by public transit. King County Metro Transit
line DART 901 has a stop approximately 100 feet north of the site entrance along 8th Avenue S
southbound.

proposal have? How many would the project or proposal eliminate?

We are writing to express our concerns with the Boys and Girls Club synthetic sports field and lighting project
located at 30815 8th Avenue South. We have enjoyed the wooded lot behind our house (one house to the left)
for 38 years, and besides the aesthetic impact to our neighborhood we have the following concerns.
This is a natural wildlife refuge for the area. Over the years, we have seen rabbits, raccoons, opossums,
squirrels, skunks, coyotes, and many bird species living in this wooded area. There are other smaller
mammals which live there also (i.e., wood rats, shrews, etc.). It is amazing how much wildlife lives in
these small woods. To take that away, would be a shame.
These woods absorb and filter our rain and snow falls which helps to refill underground aquifers. If this
is replaced with synthetic turf, it will no longer be absorbed naturally but will run downhill into the
backyards of bordering houses eventually finding its way into the sewer system. We worry about the
surface water runoff impact to these yards and homes.
• Although we don't mind the happy sound of children playing, this will be different and at a much higher
level. This is especially true if the Club plans on renting out the field to other sport entities such as adult
rec leagues which play later at night and need parking and lighting.
Our neighborhood already suffers with high noise pollution (from airplanes) which has increased over
the years to become almost unbearable at times, To add more noise and then top it off with nighttime
light pollution would definitely have a negative impact on our property value and our peace at home.
We understand this is a tricky situation since the Boys and Girls Club does own the property and probably feels
they need to develop it to better serve the community. We agree they have been good neighbors for years and
provide a wonderful service to our children. However, couldn't they use this land in a more neighborhood
friendly manner? Holding wildlife and natural science classes or even developing a small "campground"
experience are a couple suggestions.
We hope you will give our concerns your consideration and we also would like to be notified of your final
decision.

I am writing to object to this proposal of land use by The Boys and Girls Club of Federal Way, WA.
There will be significant impact on the environment in proximity to the proposed development of this
area. One of the most important will be the loss of the large, old trees on this property. The big trees,
in this wooded area, provide nesting sites, oxygen, for our air, and they absorb rain water that helps
replenish the aquifers underneath this land. Removing these trees and their complex root systems, that
stabilize the soil, would make the drainage problems in this area worse.
Along with the drainage issues, there is the impact on wildlife affected by the project: Squirrels, Hawks,
Doves, Stellar Jays, Crows, Raccoons, Skunks, Opossums, Frogs, Owls, Oregon Juncos, House Finches,
Hummingbirds, Starlings, Chickadees, Woodpeckers, (including Red -Breasted Sapsuckers, Northern
Flickers and the Downy Woodpecker), Bush Tits, Swallows, Thrushes, to name a few. This wooded area
is part of one of the last remaining refuges for this wildlife. (I have lived at 30923 5th Place South,
Federal Way, since 1984). The wildlife has retreated to this area, over the past 10 to 12 years or so,
after the building of two subdivisions cleared them out of a slightly larger area.
Not just the wildlife will be affected by this project, the effect on the surrounding residential homes will
be significant... the noise, increased traffic, and the lights (mentioned in the proposal), will not be
welcomed by the homeowners whose property borders on this area. Even though the Federal Way High
School Football Field is some distance away, we can hear the music and noise of the crowds on a game
night.
We do not need yet another Sports Field; there are no shortages of sports fields/playfields in the area.
The Boys and Girls Club is adjacent to a large, open playfield that could be utilized. Is this sports field a
necessity, or, is this just an item to put on a list of achievements to be added to someone's resume? Is
there a possible conflict of interest regarding a City Council member being a former office holder of The
Boys and Girls Club? If the Washington State Boys and Girls Club's budget is running a deficit — why are
they spending on another sports field/playfield?
After the demolition mfanempty house onthis property, why not invest inpreserving the wildness and
the educating young people about living in a habitat that includes all of the nature right around them.
Why domee°bus"youngsters tm"view and enjoy nature/mi|dness7° Why not teach the kids, and their
parents, to co -exist and enjoy the nature and wildlife, trying to live in their neighborhoods.
We should remember that we, as humans, impact everythingmround us. Development, for the sake of
development, no matter how well intentioned — has an environmental impact. VVesee this inthe
removal ofdams, once thought bmbethe thing tmdo. Removal mfthese dams isproving that some of
these ideas, and their consequences, must be un-done at taxpayers' expense.

not be missed. Cleaning up the brush and taking down the mess of
cottonwood trees (that produce a huge amount of cotton fluff every
spring) and blackberry vines is long overdue. As the closest
neighbor to the club, wtot l approve the changes, as do all the
neighbors we have talked to.

Club Sports Field Project. This project will directly impact our home as well as the value
of our home and quality of living in the home. We moved into our home about 5 years

most days and sometimes late in the evening. Most evenings seem to be adult playtime. I
am concerned that already loud play in the evenings will only increase with a new
play -field. They are proposing 30 foot tall lighting behind our bedroom window. Not
many people would welcome field lighting in their bedrooms. I am concerned that they
will be putting in a turf field as well. There are no long term studies on turf fields and the
carcinogens they emit. There may be potential risks to children who have prolonged
exposure to turf. Not sure what that might do to the air quality to homes surrounding
these types of fields. Another concern is the destruction of the trees and building will
most likely push rodents and other wildlife in the direction of our home. When the

The proposal is to construct a new all-purpose synthetic turf field (approximately 30,000 square
feet) with lights and an appropriate drainage system located at the King County Boys and Girls
Club property in the city. The affected geographic area is approximately 4.22 acres spread
across three tax parcels.
12. Location of the proposal. Give sufficient information for person to understand the
precise location of your proposed project, including a street address, if any, and section,
township, and range, if known. If a proposal would occur over a range of area, provide
the range or boundaries of the site(s).Provide a legal description, site plan, vicinity map,
and topographic map, if reasonably available. While you should submit any plans
required by the agency, you are not required to duplicate maps or detailed plans
submitted with any permit applications related to this checklist.
The location of the proposal is at 30815 8th Avenue South, Federal Way, Washington 98003.
Township::21 N, Range: 04E, Section: 08. The King County tax parcels affected by this
-proposal are 0821049058, 0821049268, and 0821049119. A site plan, vicinity map, and site
grading (current and proposed) are provided in association with submission requirements of
the permit application.

a. General description of the site (circle one): Flat, rolling, hilly, steep slopes,

Site topography generally slopes gently down toward the northwest with vertical relief across the
affected geographic area approximately 15 feet.
b. What is the steepest slope on the site (approximate percent slope)?
The steepest slope in the affected geographic area is approximately 5%.
c. What general types of soils are found on the site (for example, clay, sand, gravel, peat,
muck)? If you know the classification of agricultural soils, specify them and note any
agricultural land of long-term commercial significance and whether the proposal
results in removing any of these soils.
Exploration borings encountered native sediments consisting of medium dense grading to very
dense sand with varying silt and gravel content interpreted as Vashon lodgement till.
d., Are there surface indications or history of unstable soils in the immediate vicinity? If
so, describe.
No, there are no indications of unstable soil in the vicinity.
e. Describe the purpose, type, total area, and approximate quantities and total affected
area of any filling, excavation, and grading proposed. Indicate source of fill.
Approximately 3,250 square yards of fine grading will occur on the location of the proposed field.
Approximately 610 cubic yards of excavation will occur for the creation of a pipe trench, which
will be filled with a compact gravel base and native backfill totaling the original 610 cubic yards.
f. Could erosion occur as a result of clearing, construction, or use? If so, generally
describe.
Yes, erosion could occur during grading and the removal of vegetation in the affected
geographic area. However, the implementation of a temporary sediment and erosion control
plan using Best Management Practices should mitigate impacts.
g. About what percent of the site will be covered with impervious surfaces after project
construction (for example, asphalt or buildings)?
The total amount of impervious surface modeled for the affected geographic area is r
approximately 8,900 square feet, consisting of 25% of the synthetic field area, an ADA parking
area, pavement patching, and walking paths.
The following measures will be used to control erosion/sedimentation processes:

• Clearing Limits — All areas to remain undisturbed during the construction of the project
will be delineated prior to any site clearing or grading.
• Cover Measures — Disturbed areas shall be covered as required in Section D.4.2 of the
KCSW DM.
• Construction Entrances — A stabilized construction entrance will be used during
construction of the project.
• Perimeter Protection - Filter fabric fences for ditch and site runoff protection will be
provided at the downstream site perimeter.
• Sediment Retention — Sediment traps will be used to treat runoff before discharge to the
new drainage/dispersal system during construction.
• Storm Drain Inlet Protection Filter fabric protection will be provided on all new and
existing catch basins downstream of construction activities.
• Surface Water Control - Interceptor ditches will be used to direct runoff from construction
areas to the sediment traps and ponds.
• Dust Control — Dust control measures will be implemented when exposed soils are dry
to the point that wind transport is possible and roadways, drainage ways, or surface
waters are likely to be impacted.
2. Air
a. What types of emissions to the air would result from the proposal during
construction, operation, and maintenance when the project is completed? If any,
generally describe and give approximate quantities if known.
Construction would result in a temporary increase in air pollution, including emissions from
equipment and dust from construction activities. With adherence to applicable construction
regulations, significant air quality impacts are not anticipated. Post -construction emissions
would include emissions from vehicle trips associated with the recreational use and air
emissions associated with equipment used for field maintenance.
b. Are there any off -site sources of emissions or odor that may affect your proposal? If
so, generally describe.
Vehicle traffic along 8th Avenue S is the primary source of off -site air emissions.
c. Proposed measures to reduce or control emissions or other impacts to air, if any:
Watering the ground as needed before and during clearing and grading activities will control
dust particles. Vehicles that are not being used in construction activities will be shut off.
3. Water
a. Surface Water:
1) Is there any surface water body on or in the immediate vicinity of the site (including
year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes,

of the proposal.

the number of such systems, the number of houses to be served (if applicable), or the
number of animals or humans the system(s) are expected to serve.
There is no anticipated waste material discharge created from this proposal. Any wastewater
created because of the proposal will be mitigated in accordance with the prepared stormwater
management protection plan.
c. Water runoff (including stormwater):
1) Describe the source of runoff (including storm water) and method of collection
and disposal, if any (include quantities, if known). Where will this water flow? Will
this water flow into other waters? If so, describe.

This proposal will collect runoff and potential runoff and discharge it to two dispersal trenches
located to the north. The trenches will disperse the runoff to a native vegetated area that will be
protected as a Native Growth Protection Area. This area slopes to the north and will remain as -
is. Post Developed flows will be under 0.5 cfs and a dispersal trench will be utilized for the
outfall. Full Dispersion Trench Flow Control BMPs were sized according to KCSWDM Appendix
C.2.1.5 for the new synthetic turf field (10,000 sf of flow area per 35,000 sf nonnative pervious
surface). This 29,150 sf non-native pervious surface requires 8,835 sf of flowpath. Our 10LF
trench with 154 LF flowpath and the 44LF trench with 167 LF flowpath provide over 8,850 SF of
flowpath.

Lighting produced by the proposal will be directed at the field. Shielded, and is unlikely to affect
views or be a safety hazard on or off -site.

Outdoor lighting shall be shielded so that direct illumination is confined to the property

This report presents the results of our regulated building materials assessment of the residential
building located at 30835 8th Avenue South in Federal Way, Washington. The purpose of the
assessment was to identify potential asbestos -containing material, lead -containing coatings, PCB -
containing light ballasts, and mercury -containing components prior to demolition. The assessment
included the interior, exterior, and roof.
This assessment will assist • & Girls Club • King County with communicating the presence
of regulated building materials and the presence, location, and quantity of ACM to employees,
vendors, and contractors working in the project area and to meet the requirements for an asbestos
survey for the Puget Sound Clean Air Agency (PSCAA) and a good faith inspection as required
by Washington State Department of Labor and Industries' Division of Occupational Safety and
Health (DOSH) regulations prior to building demolition. Regulations require that a complete
copy of this assessment be • in a conspicuous location • at all times during activities that
may impact known and suspect ACM.

This assessment was conducted using a modified protocol adapted from AHERA. The protocol is
as follo r-
• Identify suspect asbestos -containing materials.
• Group materials into homogeneous sampling areas/materials.
• Quantify each homogeneous material and collect representative samples. The number of
samples collected of miscellaneous materials was determined by the inspector.
• Samples of each material were taken to the substrate, ensuring that all components and
layers of the material were included.
Sample locations are referenced on the field data forms according to sample number.
Sampling was performed by an AHERA-accredited building inspector, and the use of
proper protective equipment and procedures was followed.

2. Don protective, equipment (respirator and protective clothing if needed).
3. Label sample container with its identification number and record number. Record sample
location and type of material sampled on a sampling data form.
4. Moisten area where sample is to be extracted (spray the immediate area with water).
5. Extract sample using a clean knife, drill capsule, or cork boring tool to cut out or scrape
off approximately one tablespoon of the material. Penetrate all layers of material.
6. Place sample in a container and tightly seal it.
7. Wipe the exterior of the container with a wet wipe to remove any material that may have
adhered to it during sampling.
8. Clean tools with wet wipes and wet mop; or vacuum area with HEPA vacuum to clean all
debris.
9. Discard protective clothing, wet wipes and rags, cartridge filters, and drop cloth in a
labeled plastic waste bag.
3.3 Analytical Methodology
Suspect ACMs were sampled in general accordance with 40 CFR 763.86 by an Environmental
Protection Agency (EPA) AHERA-accredited building inspector. Each sample was collected and
stored in a heavy-duty, self-sealing plastic bag, and shipped to Seattle Asbestos Test, LLC in
Bellevue, Washington. Samples were analyzed via polarized light microscopy (PLM) in
accordance with EPA/600/R-93/116. In addition, four samples were further analyzed by PLM
point count method. Seattle Asbestos Test, LLC is accredited to perform PLM analysis by the
National Institute of Standards and Technology National Voluntary Laboratory Accreditation
Program (NVLAP).
3.4 Asbestos Results
Table 3.4-1 provides a list of suspect homogeneous sampling area (HSA) material descriptions,
material locations, and results for this sampling. Asbestos -containing materials and assumed
asbestos -containing materials are presented in bold text. Refer to the attached Figures for sample
locations and room number designations (as applicable). Refer to the attached photographs for
HSA photographs.

Additional suspect ACMs may be present in inaccessible or concealed spaces. These spaces
include, but are not limited to, electrical systems, pipe chases, spaces between
wall/ceiling/door/floor cavities, interior of mechanical components, beneath foundation pads, etc.
If future maintenance, renovation, and/or demolition activities make these areas accessible, Argus
Pacific recommends that a thorough assessment of these spaces be conducted at that time to
identify and confirm the presence or absence of additional suspect ACMs. Until then, all such
unidentified materials must be treated as assumed ACMs in accordance with applicable federal,
state, and local regulations.
If the analytical results indicate that all the samples collected per HSA do not contain asbestos,
then the HSA (material) is considered a non -ACM. However, if the analytical results of one or
more of the samples collected per HSA indicate that asbestos is present in quantities of greater
than one Cocercent asbestos as defined bri the EPA. all of
ACM regardless of any other analytical results (unless a representative number of samples have
been analyzed by PLM point counting as described below, and the results indicate the material
contains less than one percent asbestos).

4.0 LEAD ASSESSMENT
Homogeneous areas of suspected lead -containing coatings (paints) were identified and sampled in
accessible areas throughout the interior and exterior of residential building located at 30835 8th
Avenue South in Federal Way, Washington. Homogeneous painted surfaces were defined by
substrate, application, and color.
4.1 Sampling Methodology
Paint chip samples were collected to the substrate to ensure that all layers present on the substrate
were included in the laboratory analysis. Each sample was collected and stored in a heavy-duty,
self-sealing plastic bag and delivered to NVL Laboratories in Seattle, Washington. Samples were
analyzed via Atomic Absorption Spectrophotometry in accordance with Method EPA 7000B.
NVL Laboratories in Seattle, Washington is accredited by the American Industrial Hygiene
Association (AH-1A) for lead analysis.
4.2 Lead Sampling Results
Ten paint chip samples were collected and analyzed for lead. Six samples had reportable levels of
lead. The results of the analyses are presented in Table 4.2- 1.

An inventory of fluorescent light tubes, HE) lamps, and potential PCB -containing ballasts was
conducted in accessible areas of the project.
Where fluorescent light fixtures were accessible, the ballast covers were removed, and the ballast
labels were visually examined. Different types of fluorescent fixtures were distinguished by
shield shape, fixture dimension, diffuser type, and the manner in which the ballast covers were
connected to the fixture. Inspectors attempted to visually inspect at least two of each type of
fluorescent light fixture.
• Each single light tube fluorescent fixture contains one ballast.
• Each multiple light tube fluorescent fixture contains one ballast for every pair of light
tubes.
• All light ballasts are assumed to contain PCBs unless the manufacturer's label indicates
they are electronic ballasts.

6.0 CONCLUSIONS AND RECOMMENDATIONS
On December 6, 2013, Argus Pacific conducted a regulated building materials assessment of the
residential building located at 30835 8th Avenue South in Federal Way, Washington.
6.1 Asbestos
The results of the asbestos assessment conducted at the residential building in Federal Way,
Washington indicate that the following building materials sampled are ACMs or are assumed to
contain greater than one percent asbestos.

If future maintenance, renovation, and/or demolition activities make these areas accessible, Argus
Pacific recommends that a thorough assessment of these spaces be conducted at that time to
identify and confirm the presence or absence of additional suspect ACMs. Until then, all such
unidentified materials must be treated as assumed ACMs in accordance with applicable federal,

was performed using polarized light microscopy (PLM) in accordance with Test Method US EPA/600/R-93/116.
Percentages for this report are done by Visual estimate. Since variation in data increases as the quantity of asbestos
decreases toward the limit of detection, the EPA recommends point counting for samples containing between <1 %
and 10%® asbestos (NESHAP, 40 CFR Part 61). Statistically, point counting is a more accurate method. If you feel a
point count might be beneficial, please feel free to call and request one.
The test results refer only to the samples or items submitted and tested. The accuracy with which these samples
represent the actual materials is totally dependent on the acuity of the person who took the samples. This report
must not be used by the client to claim product certification, approval, or endorsement by Seattle Asbestos Test,
LLC, NVLAP, NIST, or any agency of the Federal govemment.
This report is highly confidential and will not be released without your consent. Samples are archived for two weeks
after the analysis, and disposed of as hazardous waste thereafter.

Preparation of these samples was conducted following protocol outlined in EPA Method
SW 846-3051 unless stated otherwise. Analysis of these samples was performed using
analytical instruments in accordance with U.S. EPA, NIOSH, OSHA and other ASTM
methods.
For matrix materials submitted as paint, dust wipe, soil or TCLP samples, analysis for the
presence of total metals is conducted using published U.S. EPA Methods. Paint and soil
results are usually expressed in mg/Kg which is equivalent to parts per million (ppm).
Lead (Pb) in paint is usually expressed in mg/Kg (ppm) , Percent (%) or mg/cm2 by area.
Dust wipe sample results are usually expressed in ug/wipe and uge. T.CLP samples are
reported in mg/L (ppm). For air filter samples, analyses are conducted using NIOSH and
OSHA Methods. Results are expressed in ug/filter and ug/m3. Other matrix materials are
analyzed accordingly using published methods or specified by client. The reported test
results pertain only to items tested. Lead test results are not blank corrected.
For recent regulation updates pertaining to current regulatory levels or permissible
exposure levels, please call your local regulatory agencies for more details.
This report is considered highly confidential and will not be released without your approval.
Samples are archived for two weeks following analysis. Samples that are not retrieved by
the client are discarded after two weeks.
