ELIGIBLE FACILITIES CHECKLIST
Proposed modifications to an existing tower or base station must meet at least one of the three threshold criteria
below for an eligible facilities request. For definitions please refer to FWRC 19.256.020. Check all that apply.
Collocation of new transmission equipment
Removal of transmission equipment
Replacement of transmission equipment
Is the proposed modification a substantial change as defined in FWRC 19.256.020? To determine, fill in the
appropriate information below. If the application does not meet all of the criteria below, then the project does
not qualify as an eligible facility and it is unnecessary to complete this form. To determine the correct review
process, refer to FWRC Chapter 19.256, "Wireless Communication Facilities."
Criteria 1
• For towers other than towers in the public rights -of -way, it does not increase the height of the tower by more
than 10 percent, or by the height of one additional antenna array, with separation from the nearest existing
antenna not to exceed 20 feet, whichever is greater.
■ For other eligible support structures, it does not increase the height of the structure by more than 10 percent,
or more than 10 feet, whichever is greater.

Criteria 2
• For towers other than towers in the public rights -of -way, it does not involve adding an appurtenance to the

of the tower structure at the level of the appurtenance, whichever is greater.
• For other eligible support structures, it does not involve adding an appurtenance to the body of the structure
that would protrude from the edge of the structure by more than six feet.

Crown Castle USA Inc. ("Crown Castle") on behalf of T-Mobile West LLC ("T-Mobile") is submitting the attached
Eligible Facilities Request application to modify transmission equipment on a telecommunications tower located at
31617 FIRST AVENUE SOUTH, FEDERAL WAY, WA 98003 in CITY OF FEDERAL WAY, WA (the "MIRROR LAKE
Tower").
Section 6409 of the Middle Class Tax Relief and Job Creation Act of 2012, commonly known as the "Spectrum Act"
(Pub. Law No. 112-96, 126 Stat 156), mandates that state and local governments "may not deny, and shall approve, any
eligible facilities request for a modification of an existing wireless tower or base station that does not substantially
change the physical dimensions of such tower or base station." Additionally, if "the reviewing State or local government
determines that the application is incomplete" [they] "must provide written notice to the applicant within 3o days of
receipt of the application, clearly and specifically delineating all missing documents or information." Under federal law,
an Eligible Facilities Request is deemed granted with written notification in sixty (6o) days after an application is filed
with a local jurisdiction, excluding tolling.
T-Mobile proposes to modify the "MIRROR LAKE Tower" as follows:
Replace (3) antennas and (3) RRU's; and install (1) FXFC, (1) hybrid cable, and (1) junction box on existing
tower. T-Mobile also proposes to replace (1) cabinet; remove (1) cabinet and (4) baseband units; relocate (2)
baseband units; and install (8) modules and (1) junction box within existing compound.
