The District has received all closing documentation and has completed a satisfactory final
inspection of the referenced project for compliance with Lakehaven Utility District
standards for materials and construction of sewer systems. Lakehaven hereby initially
accepts this project for ownership, operation and maintenance.
The Maintenance Bond (No. SG 6003) submitted for the project in the amount of
$12,000.00, shall be maintained in force for a period of two years from this date of initial
acceptance and until the District gives written notice of its release, pending satisfactory
results of a final inspection to ensure the systems remain in compliance with Lakehaven's
standards for materials and construction.
The Restoration Bond(No. SG 6003) held to guarantee restoration of affected rights-of-way
and/or off-site easements is hereby satisfied and may now be released. The bond was issued
by CBIC in the amount of $4,000.00. A copy of this letter should be sufficient
authorization to the surety to effect its release.
The District has reviewed the project account and has estimated the anticipated expenses
needed to complete the project through the two-year warranty period. If the actual costs
exceed the amount held on deposit following completion of the two-year warranty period,
you will be invoiced accordingly. The invoice must be paid prior to release of the
Maintenance Bond. Any funds remaining in the account will be refunded following the
satisfactory completion of the two-year warranty period, as evidenced by a successful
inspection of the systems by District personnel, at which point the project will be granted
final acceptance by Lakehaven.

Dear Mr. Nog,
The Public Works Department has reviewed the engineering plans for the street and storm drainage
improvements associated with the above-referenced project. Prior to approval of these plans, the
following items must be addressed:
All Sheets:
Add the engineering(EN)approval number(noted above)just above the approval block.
Sheet 1:
1) The drawing index indicates there are five sheets included in the plan set. Please remove this
reference,or update the sheet name, if additional sheets are added.
2) Provide approximate cut and fill earthwork quantities required for the street improvements.
Sheet 2:
Remove the Filter Fabric CB Protection portion from detail 1. The City allows only the CB inserts
for use in public right-of-way.
Sheet 3:
1) Provide a street cross-section detail for each of 34th Avenue SW and SW 342nd Street. The
sections shall include, but not be limited to, all existing and proposed improvements (including
pavement widths, planter strips, sidewalks, etc.), existing and proposed rights-of-way/property
lines, all existing and proposed horizontal dimensions, and proposed pavement and subgrade
thicknesses. (For your information, the pavement/subgrade thickness is: 3 inches CL B A.C.
paving,2 inches C.S.T.C.,and 5 inches C.S.B.C.).

2) A City standard barricade is required at the east end of SW 342nd Street. Show the barricade on
the plan view,and provide a detail.(City Standard drawing 3-15).

3) A sign must be attached to the barricade for notification purposes that the road is planned for
future extension. Please see the attached for further sign information.
4) The plans indicate `existing' and 'new' street crown locations on 34`"Avenue SW. However, the
paving plan only indicates new paving being constructed from the existing east edge of pavement.
Please provide additional information as necessary. (This comment may be immaterial, based on
comment#5, under the heading`Other', below.)
Sheet 4:
1) Provide individual roof drain downspout stub-outs from the storm system to each lot. This
system must be connected to catch basins. Provide the pipe size, type and slope, and include lot
stub invert elevations.
2) Number each new catch basin,and provide catch basin type.
3) All new catch basins, unless otherwise noted, must be provided with vaned grates. Please
indicate this on the plans.
4) It appears the new catch basin, located in the gutterline, at the southeast corner of 34th Avenue
SW and SW 342nd Street,should be configured as a`through-curb' inlet,based on it being located
near the sag point of the vertical curve. Please revise, or provide information supporting a
standard catch basin frame at this location.
Other:
1) Provide a Technical Information Report (T.I.R.) for the project. Please call or e-mail to discuss
the information required as part of the T.I.R.
2) Add City Standard detail drawings 3-12 (sidewalks) and 3-6 (residential driveways) to the plan
set.
3) SW 342nd Street dedication shall be 32-feet, to accommodate the required three-foot utility strip
behind sidewalk. Revise the plans to show this. This shall also be revised on the Short Plat
documents.
4) Street trees are required within the new planter strips. Utilizing the enclosed City details, show
the location and species of each street tree. Include detail 3-29 and 3-33 on the plans. Please
note that the street trees must be minimum 2112"caliper.
5) It appears 34th Avenue SW will be open-cut in order to provide water service to the lots, and
potentially to provide other dry utilities. If so, 34th Avenue SW is subject to a full-width asphalt
overlay. The overlay limits will need to be shown on the plans. Grinding of the existing asphalt
is required along the existing concrete gutters, and a butt joint required at the southerly/westerly
extent of overlay on SW 343rd Street. The required pavement thickness for the overlay is 1 %2
inches Class A.

6) Bonding is required for all street and storm drainage improvements. Please complete and return
the enclosed Bond Quantities Worksheet. Please also provide the applicant with the enclosed
Bond Information sheet, for its completion and return to the City. (An Assignment of Funds in
Lieu of Bond may be utilized, if preferred.)
7) Please refer to the red-lined plans for additional comments not covered under this letter.
8) Additional items, as requested above, may be added to a separate detail sheet,.or added (and
referenced) to existing sheets,as necessary. The City details are available for download from our
website at www.cityoffederalway.com.
