Hi Becky,
This is Eric Bitterman. Thank you for returning my phone call about getting trees removed in our neighborhood
of Brighton Park. I have attached some documents that I hope will help answer our question as to any permits
or rules for removing some hazardous tress in Tract E of our neighborhood. Among the documents will find our
neighborhood layout with Tract info, the evaluation of the trees in question, and the "Request for
Administrative Decision" report. Please let me know if you need anything else, or if there is any other
documentation that is needed.

TERMS AND CONDITIONS:
Estimate valid for 30 days. All tree work is to be completed in a workman like manner. Apex Tree Experts assumes no responsibility for damage
to underground installations:, such as, but not limited to pipes, cables, wires, irrigation systems, septic systems and utilities not clearly marked
by the site owner prior to the beginning of work. Additional costs may be incurred if metal or concrete are found which result in damage to
stump grinders, chain saws, chain saw bars or chains. All dog feces must be removed from work area prior to the date of service or S150.00 Fee
will apply. Contracted stumps will be ground 4-6 inches below the surface. Stump grindings will be left as mulch. Apex Tree Experts is not
responsible for any loss, damage, delay, or interruption caused by Weather conditions, acts of God, neglect of customer, or other causes beyond
Apex Tree Experts control. Customer shall indemnify Apex Tree Experts against any losses, damages, legal fees, and other expenses Apex Tree
Experts may suffer or incur by reason of the breach of any of the agreements, or representations made in the contract by customer.
Payment is due in full upon completion of work unless otherwise noted above. Accounts unpaid after 10 days are subject to a S50 late fee. In
the event that a delinquent account is placed in the hands of an attorney or licensed collector for collection, customer agrees to pay, in addition
to the amount of said account and interest at 1.5% per month, an additional late fee of $50 and all cost of collections, including attorney's fees.
Return check fee is $75.There is a 2% credit card fee.
I certify that I am the owner, or have the legal tight to represent the owner, of the above property, trees, and landscape and indemnify Apex
Tree Experts for all damages, attorney's Fees and cost should a legal dispute arise concerning the ownership of the above property, trees, and
landscape. Cancellation subject to a cancellation fee of 25% of cite contracted price. Signatures below will constitute a binding contract. The
above prices and specifications are satisfaclory and hereby accepted. Apex Tree Experts is authorized to do the work as specified.
