Dear Mr. Follansbee.
The City of Federal Way has completed administrative review of the above -referenced Boundary Line
Adjustment (BLA) application. The proposal is to remove the interior lot line between tax parcels
768190-0010 and 768190-0020. Zoning for both parcels is Community Business (BC). The application
was deemed complete on August 8, 2016.
The Department of Community Development hereby approves the BLA based on the survey drawing
prepared by AHBL, resubmitted on October 13, 2016.
DECISIONAL CRITERIA
City staff has determined the BLA meets decisional criteria set forth in Federal Way Revised Code
(FWRC) Chapter 18.10:
1. The BLA is a minor alteration in the location of lot boundaries on existing lots. The proposal is to
remove the interior lot line between tax parcels 768190-0010 and 768190-0020. The BLA will not
increase the number of lots, nor diminish the size of open space or other protected environments.
There is a Type Ns stream located on the site. The stream and 35-foot stream buffer is depicted on the
BLA drawing.
2. The BLA will not diminish the size of any lot so as to result in a lot of less square footage than
prescribed in the BC zoning district. FWRC Chapter 19.220 does not contain prescriptive lot size
minimums for the proposed facility.
3. The BLA will not result in the reduction of setbacks or site coverage to less than prescribed in the BC
zoning district. The BLA is required to prevent the proposed structure from being built over a
property line. No site (lot) coverage limitations are prescribed for this use in the BC zone.
4. All lots resulting from the BLA will be in conformance with the design standards of the city's
subdivision title.

REVISIONS TO BLA DRAWING
The following comments must be revised on the final BLA drawings prior to recording.
• Please remove the King County Health Department signature block on Sheet 1.
Please provide formal documentation that Leslie Davis SVP, CFO, Telecare Corporation, has the
signature authority for this document. The email provided is insufficient.
RECORDATION
Pursuant to FWRC 18.10.060, all boundary line adjustments shall be recorded with the King County
Division of Elections and Records. To finalize the BLA process, please use the enclosed Resubrniltal
li7forrnation form and submit to the Permit Center one signed and notarized drawing (paper or mylar), two
paper copies of the signed/notarized drawing, and payment for recording fees in the amount of $163.00.1
If you are providing payment with a check, please make it payable to City of'Fecleral Way. The City will
sign the BLA document and have it recorded at King County at the expense of the applicant. You will be
provided a conformed copy of the recorded BLA.
Please hole: Blue ink is not acceptable on map documents; all signatures and writing must be made with
permanent black ink.
CLOSING
This land use decision does not waive compliance with future City of Federal Way codes, policies, and

Dear Mr. Follansbee:
The City of Federal Way's Department of Community Development is in receipt of the above -referenced
Boundary Line Adjustment (BLA) application. The applicant proposes to remove the interior lot line
between tax parcels 768190-0010 and 768190-0020. Pursuant to Federal Wav Revised Code (FWRC)
18.10.040, within 28 days of receiving a BLA application the city shall determine whether all information
and documentation required for a complete application has been submitted.
NOTICE OF COMPLETE APPLICATION
The BLA application was deemed complete as of August 8, 2016. This determination of completeness is
based on a review of your submittal relative to applicable requirements referenced within FWRC Chapter
18.10 "Boundary Line Adjustments."
A 120-day time line for reviewing the BLA application has started as of this date. The city's development
regulations allow the department 120 days from the date that an application is deemed complete to take
action on the application. However, the 120-day time line can be stopped any time the city requests
corrections and/or additional information.
TECHNICAL COMMENTS
During the course of complete application review, staff completed technical review of your application.
The following items must be addressed prior to the department issuing approval of the BLA application.
1. Show the City project number, 16-103522-00-SU, in the upper left corner of both sheets.
2. Use the City's most current BLA title block.
3. Show the recording numbers of all easements on the plans.
4. Verify that all easements listed in the title report are shown on the plans.
5. Names of the vested owner and signators must be shown in the Authorization on sheet 1,

6. For all signators, provide proof of authority to sign on behalf of the LLC.
7. The PLS signature must appear on both sheets.
8. Indicate that the `old foundation' and `edge of asphalt' are to be removed.
9. In December 2015, the applicant submitted a Critical Area Report/Stream Determination for the
subject property, prepared by Sewall Wetland Consulting Inc. (dated December 7, 2015.)
According to that report the site contains a Type Ns stream, which requires a 35-foot buffer. This
was confirmed by the City's Consultant, Otak. Please depict the Type Ns stream and 35-foot
buffer on the BLA drawing.
CLOSING
Once the minor revisions above are made, please provide two paper copies of the BLA document with the
enclosed resubmittal form to the Permit Center.
Pursuant to FWRC 18.05.080, if an applicant fails to provide additional information to the City within
180 days of being notified that such information is requested, the application shall be deemed null and
void and the City shall have no duty to process, review, or issue any decisions with respect to such an
application.

COMMITMENT
Conditions and Stipulations
1. The term "mortgage" when used herein shall include deed of trust, trust deed, or other security
instrument.
2. If the proposed Insured has or acquires actual knowledge of a defect, lien, encumbrance,
adverse claim or other matter affecting the estate or interest or mortgage thereon covered by
this Commitment, other than those shown in Schedule B hereof, and shall fail to disclose such
knowledge to the Company in writing, the Company shall be relieved from liability for any loss or
damage resulting from any act or reliance hereon to the extent the Company is prejudiced by
failure to so disclose such knowledge. If the proposed Insured shall disclosure such knowledge
to the Company, or if the Company otherwise acquires actual knowledge of any such defect, lien,
encumbrance, adverse claim or other matter, the Company at its option, may amend Schedule B
of this Commitment accordingly, but such amendment shall not relieve the Company from liability
previously incurred pursuant to paragraph 3 of these Conditions and Stipulations.
Liability of the Company under this Commitment shall be only to the named proposed Insured
and such parties included under the definition of Insured in the form of Policy or Policies
committed for, and only for actual loss incurred in reliance hereon in undertaking in good faith
(a) to comply with the requirements hereof, or (b) to eliminate exceptions shown in Schedule B,
or (c) to acquire or create the estate or interest or mortgage thereon covered by this
Commitment. In no event shall such liability exceed the amount stated in Schedule A for the
Policy or Policies committed for and such liability is subject to the Insuring provisions, exclusion
from coverage, and the Conditions and Stipulations of the form of Policy or Policies committed for
in favor of the proposed Insured which are hereby incorporated by references, and are made a
part of this Commitment except as expressly modified herein.
4. Any claim of loss or damage, whether or not based on negligence, and which arises out of the
status of the title to the estate or interest or the lien of the Insured mortgage covered hereby or

The term "mortgage" when used herein shall include deed of trust, trust deed, or other security
instrument.
If the proposed Insured has or acquires actual knowledge of a defect, lien, encumbrance,
adverse claim or other matter affecting the estate or interest or mortgage thereon covered by
this Commitment, other than those shown in Schedule B hereof, and shall fail to disclose such
knowledge to the Company in writing, the Company shall be relieved from liability for any loss or
damage resulting from any act or reliance hereon to the extent the Company is prejudiced by
failure to so disclose such knowledge. If the proposed Insured shall disclosure such knowledge
to the Company, or if the Company otherwise acquires actual knowledge of any such defect, lien,
encumbrance, adverse claim or other matter, the Company at its option, may amend Schedule B
of this Commitment accordingly, but such amendment shall not relieve the Company from liability
previously incurred pursuant to paragraph 3 of these Conditions and Stipulations.
Liability of the Company under this Commitment shall be only to the named proposed Insured
and such parties included under the definition of Insured in the form of Policy or Policies
committed for, and only for actual loss incurred in reliance hereon in undertaking in good faith
(a) to comply with the requirements hereof, or (b) to eliminate exceptions shown in Schedule B,
or (c) to acquire or create the estate or interest or mortgage thereon covered by this
Commitment. In no event shall such liability exceed the amount stated in Schedule A for the
Policy or Policies committed for and such liability is subject to the Insuring provisions, exclusion
from coverage, and the Conditions and Stipulations of the form of Policy or Policies committed for
in favor of the proposed Insured which are hereby incorporated by references, and are made a
part of this Commitment except as expressly modified herein.
4. Any claim of loss or damage, whether or not based on negligence, and which arises out of the
status of the title to the estate or interest or the lien of the Insured mortgage covered hereby or
any action asserting such claim, shall be restricted to the provisions and Conditions and
Stipulations of this Commitment.
