House built in 1965 and ADU in previous cabin's footprint in 2016 Gazebo with kitchen in 2016 and shoreline exemption for
retaining wall in 2017. No habitat assessment was completed since there were no habitats affected. Property exists on erosion
hazard area.
If the value of an addition or alteration to a structure within an area designated as a "special flood hazard" equals or exceeds
50% of the value of the structure before the addition or alteration, the entire structure must be treated as a substantially
improved structure. The following information must be provided if the proposed improvement is within a "special flood
hazard" zone identified by FEMA.

the agencies to which this application is made, the right to enter the above -described location to inspect the proposed and or
completed work.
Warning and Disclaimer of Liability
The review of your development is solely for the purpose of determining if your application conforms with the written
requirements of the floodplain regulations. It is not to be taken as a warranty. Compliance with the regulation does not
insure complete protection from flooding. FWRC Chapter 15.10 meets established standards for floodplain management,
but neither this review nor the regulation take into account such flood related problems as natural erosion, streambed
meander or man-made obstructions and diversions all of which may have an adverse effect in the event of a flood. The
owner is advised to consult an engineer or other expert regarding these considerations.
In consideration for the issuance of the requested permit the applicant, owner, agent, engineer and their successors agree to
hold the City harmless from any onsite or offsite damages of any kind arising from the development of the subject property
in accordance with their submittals as outlined in their eventual permit.
All correspondence and notices will be transmitted to the Land Owner of Record and copies sent to the authorized
agent, as applicable. By signing this document, I agree that all information submitted is true and correct, and that I
have read and understand the above warning and disclaimer of liability.

Dr. Shaw;
On April 20, 2016 you received permission to replace a free-standing Accessory Dwelling Unit (ADU),
and on April 20, 2016 obtained a permission to build a gazebo, both structures on your property at 30020
20`h Pl. SW behind an existing retaining wall. You followed construction conditions provided by the
City, and completed the structures.
On review of the permit, it is noted that a Flood Permit was also necessary to have the buildings
approved. There was no flood permit issued. I have enclosed a completed application requesting
approval of a Flood Permit for the construction of the cabana and ADU. I have completed the necessary
application with the relevant information to be placed within the file showing that a flood permit can be
issued for the construction.
We need your signature on this application. Once we receive this signed application, I will issue the
permit and your file here and at the State will be properly completed. Please note that this will not cost
you money and no additional work has to be done regarding these two structures. The purpose of this
application and flood permit issuance is to close the file in our records, and to close the file for state
records to show that appropriate process has been completed to construct the buildings.
I ask that you call me if you have any questions regarding this letter or application at 253-835-2643. If
you have no questions, please sign the application and return it to the City of Federal Way with attention
to me, and I will send you a letter of approval of the flood permit. I appreciate your response in this
