1. Seaalon B/S•-1 indicates #4 hooked bars starting below the continuous footing reinforcement.

minimum embedment of 8 incdtes is provided, in azcordance with the attached sketch.
2. The foundation plan for building A is incomplete as no provision for the interior shear walls
has been made. Provide thickened slabs as noted on the attached sketch. Anchor bolts and
hold downs are still to be provided in accordance with the design drawings.

1) $25,400.00 traffic prorata share due before permit issuance, lot
line elimination required before permit issuance, and lake # 6 and
dam construction must be completed before a final buidling
inseopction or C/O is issued. db
2) The following conditions shall apply:
1. Groundcover throughout the site shall be planted and spaced to
result in total coverage within three years.
2. Deciduous trees shall be 1.5 inches measured four and one-half
feet above the rootball at the time of planting.
3. In accordance with FWCC section 22-1568(6)(a through g), the
two significant trees to be retained shall be surrounded by a no
disturbance area which shall consist of either a temporary five
foot chain link fence or a line of five foot high, orange colored
two by four inch states placed no more than 10 feet apart
connected by a highly visable surveyor's ribbon installed around
the drip line of the trees. These shall be in place before
clearing or grading activities begin. No impervious surfaces,
fill, excavation or storage of construction materials shall be
permitted within the no -disturbance area.
4. If either of these trees are damage or destroyed, then two ten
foot tall replacement trees shall be installed per FWCC.
5. 2 Acer circinatum (Vine maple) trees proposed in the parking
lot landscape island SE of building C shall be eliminated as noted
on the approved landscape plan due to the underground storm system
pipes.
6. One Acer rubrum Bowhall (Bowhall Columnar Maple) shall be
installed 10 feet east of the proposed planting area in the
northern most parking lot due to underground storm pipes per the
approved landscape plan.
7. Nine (9) photinia shrubs (instead of 6) shall be planted on
four foot centers to achieve parking lot screening. These shall be
30 inches tall when planted.

3) Pursuant to to site plan review condition #2, Lake #6 expansion
and subsequent dam construction shall be completed on or BEFORE
October 1, 1996. If the lake construciton is not completed by this
date, ALL WORK ON THE 348TH PROFESSIONAL BUILDING SITE IS TO CEASE
UNTIL THE LAKE CONSTRUCITON IS COMPLETED. There is to be no final
inspection or the certifiacte of occupancy (C/O)is issued for any
of the 348th Professional Buildings without Lake #6 expansion
completed. See Public Works with any questions. DB

4) A delineated pedestrian walkway shall be provided between
building B and C in accordance with the approved site plan. db
5) The gate of the dumpster shall be a solid site -obscuring
material. Wood slats will not suffice. db
6) The required landscaping shall be installed and the planner must
inspect and approve the required landscpaing before a final
building inspection can be scheduled or the Certifiacte of
