Why I would like to serve on the Arts Commission. I feel that my skills and assets in working with and being involved in the art community would be a valuable asset to the Arts Commission. I Enjoy working with others to reach a common goal of enhancing the quality and value of various arts in our community. What are your prior experiences in this area? My experience in the arts began as a Trumpet player in school. Then on to Drum and Bugle Corp. in the U.S. Air Force. After the Air Force my job is directly working with artist in creating art pieces across the US as a metal artist. Working with K. C. 4 Culture and their Art recognition program. How do you see your role as a Board Member/Commissioner? Working with others on the commission to improve our cultural art opportunities within our city. It takes a group of individuals all with great knowledge for this to happen. I would like to become part of that team. Opportunities may arise with connections I have in the Art World. Describe anything in your past or recent activities perceived as an embarrassment: Just recently having a lengthy conversation with a person and asking questions. Only to realize at the end of the conversation that I had mistaken who I was talking with and had actually called him by the other persons name. Fortunately the topic of discussion was very generic. If requested, they will be provided without redaction or notice. Board and Commission Application Page ) of 3 November 2013 Why do you want to serve on this Board/Commission? I want to serve on this board because I have a passion to assist those in need. Having immigrated from South Korea at a young age, I have,on countless occasions, recognized the value and importance of a simple helping hand.As such,I feel that by serving on this board, I will be able to give back to a community in the same way that my many different communities have helped me throughout the years. What are your experiences in this area? I am fluent in both English and Korean and have therefore used this skill throughout the last few years in various fields. From 2002-2010, I have served as an interpreter for both the Federal Way Municipal Court as well as the Auburn Municipal Court. In 2004, I have been involved in a Korean school of language and culture through the Federal Way Central Presbyterian Church, which from 2011,expanded to the Korean School of the Greater Federal Way. How do you see your role as a Board Member/Commissioner? I view my role as a board member to be an incredible opportunity to combine both my passions to help and my ability to do so. Community and service,to me,are inseparable concepts that must go hand in hand. My ninth grader attends FWPA,in which I have been an active member of the PTSA. Furthermore, I have also been volunteering, for the last 6 months,at Cornerstone Medical Services as an in-person assistor for Obamacare services. I feel that my role as a board member, therefore, would give me an ability to reach out and assist an even larger group of people. Describe anything in your past of recent activities perceived as an embarrassment: I have in the past volunteered at the Hanwoori festival and both times, due to lack of planning and resources, I felt somewhat embarrassed at the outcome. http://cmsclinic.org Suzanne.Pak@cmsclinic.org CORNERSTONE Phone: 253.252.2095 Fax: 253.252.2469 MEDICAL SERVICES Address: 1014 S.320th Street,Federal Way,WA 98003 Letter of Support for jenny Chang's Appointment to Federal Way Human Service Commission To: Denise Catalano &Carol McNeilly, City of Federal Way Address: 33325 8th Avenue S,Federal Way,WA 98003 Phone/ Email: 253-835-2651/ Denise.Catalano @cityoffederalway.com Date: March 14, 2014 M , Dear Federal Way Human Service Commission, It is with great pleasure that I recommend jenny Chang to serve on the Federal Way Human Service Commission. As the Chief Operating Officer of Cornerstone Medical Services (CMS), I have been profoundly impressed by Jenny's deep commitment to volunteerism, serving the under-served,and reaching out to minorities and immigrants. Jenny serves our community at least 10 hours a week in the following capacity: Volunteer care coordinator at the CMS free clinic, which serves over 1,500 uninsured, low income patients a year, many of whom reside in Federal Way r Volunteer In-Person Assisters for the Washington State Health Benefit Exchange (CMS was awarded the highly competitive contract in November 2013 to help enroll families into the Affordable Care Act-and I serve on the HBE's Outreach Technical Advisory Committee) r Volunteer outreach coordinator for Medicare events at Valley Cities and Mercy Housing (CMS received small event grants from Washington State Office of Insurance Commission to host Medicare seminars for minority and immigrant seniors). r Patient Advisory member for"Making Stomach Cancer a Health Priority for Asian Americans." CMS received grants from Patient Centered Outcomes Research Institute, Fred Hutchinson Cancer Research Center, and Korean General Consulate to partner with community organizations to improve early screening for stomach cancer. And we will host a summit in Federal Way in August, with help from Jenny and other advisory members. Volunteer parent coordinator at the Federal Way Korean Language Academy, which is operated by Federal Way Korean American Association and the former mayor of Federal Way Michael Park. As a Seattle King County Aging& Disability Advisory Councilmember, I am aware of the difference that a minority or immigrant representative can make, in bringing fresh perspective to the challenges and solutions to serving the under-served. Jenny is passionate about connecting individuals and organizations to each other, and lowering barriers to access to health and human services. Please feel free to contact me to request any additional information about Jenny Chang's background or qualifications. Thank you. Human Services Commission Interview Questions 1.
