MOLD INSPECTION Working Together for Quality
Date: December 12, 2014
Project No. 3128 RECEIVED
Wayne Newman- Redwood Construction JAN 0 2 2015
Site: 1900 SW Campus Dr4. #7-101 CITY OF FEDERAL WAY
Federal WA,WA 98023 CDS
1-971-533-4847
At your request I conducted a post remediation assessment of apt. #7-101,at the
above address. A prior inspection of the site indicated elevated levels of mold as
determined by direct examination of a swab sample followed by air testing.
During the site visit on the above date I collected two air samples which were
analyzed by EMLab in Bothell.Their report, attached, indicated that the levels of
airborne mold in the air inside the apartment after remediation were lower than the
levels found outside at the same time. This indicates an optimal condition.
The air testing along with moisture testing and the visual inspection strongly
indicates that the remediation efforts have been successful and the apartment is
ready for re-construction
If you have any questions, please don't hesitate to call.
Sincerely,
Robert S. Peterson

Comments:
Spore types listed without a count or data entry were not detected during the course of the analysis for the respective sample.
t The spores of Aspergillus and Penicillium(and others such as Acremonium,Paecilomyces)are small and round with very few distinguishing
characteristics. They cannot be differentiated by non-viable sampling methods. Also, some species with very small spores are easily missed, and
may be undercounted.
ttBackground debris indicates the amount of non-biological particulate matter present on the trace(dust in the air)and the resulting visibility
for the analyst. It is rated from 1+(low)to 4+(high).Counts from areas with 4+background debris should be regarded as minimal counts and
may be higher then reported. It is important to account for samples volumes when evaluating dust levels.
The analytical sensitivity is the spores/m3 divided by the raw count. The limit of detection is the analytical sensitivity multiplied by the sample
volume divided by 1000.
For more information regarding analytical sensitivity, please contact QA by calling the laboratory.
$A"Version"indicated by-"x"after the Lab ID#with a value greater than 1 indicates a sample with amended data. The revision number is
reflected by the value of"x".
§Total Spores/m3 has been rounded to two significant figures to reflect analytical precision.

*The Friedman chi-square statistic is a non-parametric test that examines variation in a set of data(in this case, all indoor spore
counts). The null hypothesis(HO)being tested is that there is no meaningful difference in the data for all indoor locations. The
alternative hypothesis(used if the test disproves the null hypothesis)is that there is a difference between the indoor locations.
The null hypothesis is rejected when the result of the test is greater than the critical value. The critical value that is displayed is
based on the degrees of freedom(dF)of the test and a significance level of 0.05.
**An agreement ratio is a simple method for assessing the similarity of two samples(in this case the indoor sample and the
outdoor summary)based on the spore types present.A score of one indicates that the types detected in one location are the same
as that in the other.A score of zero indicates that none of the types detected indoors are present outdoors.Typically,an
agreement of 0.8 or higher is considered high.

At your request I conducted a post remediation assessment of apartment#7-102 at the
above address. Elevated moisture in primarily the water heater closet had led to the growth
of mold in that area. A qualified mold remediation company has addressed the cause and
has effectively dried and cleaned the area.
As part of the post remediation assessment air samples are collected, using IESO standards
and protocols, inside containment to quantify spore counts and outside to be used as a
baseline/control for comparison. The optimal condition exists when the inside sample
spore counts of the significant molds is the same as or lower than the outside sample, In
this sample in apartment#7-102 the most favorable condition exists as the inside spore
counts are lower than the outside.
The air testing along with the moisture testing and visual inspection strongly indicates that
the remediation efforts have been successful and the apartment is ready for re-construction.
If you have any questions, please don't hesitate to call.

§TOTAL SPORES/m3 4,900
tThe Typical Outdoor Data represents the typical outdoor spore levels for the location and time frame indicated. The last column represents
the frequency of occurrence. The very low, low, med, high, and very high values represent the 10,20,50,80,and 90 percentile values of the
spore type when it is detected. For example,if the frequency of occurrence is 63%and the low value is 53,it would mean that the given spore
type is detected 63%of the time and, when detected,20%of the time it is present in levels above the detection limit and below 53 spores/m3.
These values are updated periodically, and if enough data is not available to make a statistically meaningful assessment,it is indicated with a
dash.
§Total Spores/m3 has been rounded to two significant figures to reflect analytical precision.
*The spores in this category are generally capable of growing on wet building materials in addition to growing outdoors.Building related
growth is dependent upon the fungal type, moisture level, type of material, and other factors.Cladosporium is one of the predominant spore
types worldwide and is frequently present in high numbers.Penicillium/Aspergillus species colonize both outdoor and indoor wet surfaces
rapidly and are very easily dispersed.Other genera are usually present in lesser numbers.
**These fungi are generally not found growing on wet building materials. For example, the rusts and smuts are obligate plant pathogens.
However,in each group there are notable exceptions. For example, agents of wood decay are members of the basidiomycetes and high counts of
a single morphological type of basidiospore on an inside sample should be considered significant.

Comments:A)Analysis of replicate sample is delayed.B) 12 of the raw count Cladosporium spores were present as a single
clump.Analysis of replicate sample is delayed.

***The Spearman rank correlation is a non-parametric test that examines correlation between two sets of data(in this case the
indoor location and the outdoor summary). The null hypothesis(HO)being tested is that the indoor and outdoor samples are
unrelated. The alternative hypothesis(used if the test disproves the null hypothesis)is that the samples are similar. The null
hypothesis is rejected when the result of the test is greater than the critical value. The critical value that is displayed is based on
the degrees of freedom(dF)of the test and a significance level of 0.05.
****Mo1dSCORETM is a specialized method for examining air sampling data.It is a score between 100 and 300,with 100
indicating a greater likelihood that the airborne indoor spores originated from the outside, and 300 indicating a greater likelihood
that they originated from an inside source. The Result displayed is based on the numeric score given and will be either Low,
Medium,or High, indicating a low, medium,or high likelihood that the spores detected originated from an indoor source.EMLab

MOLD INSPECTION
November 26, 2014
RECEIVED
Project No. 3117
Wayne Newman- Redwood Construction JAN 0 2 2015
1900 SW Campus Dr#7-201 CITY OF FEDERAL WAY
Federal Way,WA,98023 CDS
1-971-533-4847
At your request I conducted a post remediation assessment of apartment# 7-201 at the
above address.
The optimal condition exists when the inside sample
spore counts of the significant molds is the same as or lower than the outside sample. In
this sample in apartment#7-201 the most favorable condition exists as the inside spore
counts are lower than the outside.
The air testing along with the moisture testing and visual inspection strongly indicates that
the remediation efforts have been successful and the apartment is ready for re-construction.
If you have any questions, please don't hesitate to call.
Sincerely,
Robert S. Peterson,

Comments:A)Analysis of replicate sample is delayed.B) 12 of the raw count Cladosporium spores were present as a single
clump.Analysis of replicate sample is delayed.
For more information regarding analytical sensitivity, please contact QA by calling the laboratory.
t A"Version"indicated by-"x"after the Lab ID#with a value greater than 1 indicates a sample with amended data. The revision number is
reflected by the value of"x".
§Total Spores/m3 has been rounded to two significant figures to reflect analytical precision.

tThe Typical Outdoor Data'represents the typical outdoor spore levels for the location and time frame indicated.

The Result displayed is based on the numeric score given and will be either Low,
Medium,or High, indicating a low, medium,or high likelihood that the spores detected originated from an indoor source.EMLab

that we typically detect the given spore type on 63 percent of all outdoor samples and, when detected,2.5%of the time it is
present in levels below 53 spores/m3.

RE:Bulk Asbestos Fiber Analysis; Batch 114-1613 CDS
Dear Jason '
Thank you for choosing NOVO Laboratory and Consulting,Inc.as your laboratory. Enclosed you will find test results for the
bulk samples'submitted to the laboratory. The samples were examined for the prescence of identifiable asbestos fibers using
polarized light microscopy(PLM)with dispersion staining in accordance with the U.S.EPA Method 600/R-93/166,"Method for
determination of asbestos in Bulk Building Materials."
The samples containing more than one separable layer were separated and given an individual result,as required by the
National Emission Standard for Hazardous Air Pollutants(NESHAP,40 CFR-Part 61)protocol that multi-layered samples be
analyzed and reported separately. Asbestos concentration in samples is determined by visual estimation and reported by
percent on a volume basis.
For samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a
procedure known as point counting. Point counting is a statistically more accurate means of quantification for samples with
low concentrations of asbestos. An alternative method also recommended by the EPA is to have the sample analyzed by
transmission electronic microscopy methods. If you would like us to follow any of the recommendations listed above, please
contact us.
For samples containing vermiculite, prior to its close in 1990,much of the world's supply of vermiculite came from a mine
near Libby,Montana. This mine had a natural deposit of asbestos which resulted in the vermiculite being contaminated with
asbestos.Attic insulation produced using vermiculite ore, particularly ore that originated from the Libby mine, may contain
asbestos fibers.Due to inconsistencies in the levels of contamination of vermiculite and limitations in the current best practice
analytical methods the EPA recommends that vermiculite be presumed to contain asbestos.
This report is confidential and will not be released without prior written approval. Samples are archived for Fifteen(15)days
following analysis. Please contact us if samples need to be archived longer than the standard holding time.
The information you receive relates only to the items tested. Accuracy of the results is limited by the methodology and
expertise of the sample collector. The analyses at NOVO Laboratory and Consulting,Inc.are cross-checked with other
laboratories for quality assurance purposes.
Thank you for using NOVO Laboratory and Consulting,Inc. If you have any questions please don't hesitate to call.

At your request I conducted a limited inspection of a closet area across from the
water heater closet within apartment#7-201 at the above site. Apartment#7-102
was inspected and no areas of concern were seen as the apartment had been
painted and cleaned. In #7-201,water from a plumbing leak had damaged the
sheetrock and visible mold was present in the back of the partially demolished
closet Across from the closet the wall had been cut up about 8"up from the floor
which exposed visible mold on the exposed sheetrock wall. This area is behind the
bath tub in the adjacent bathroom. There is visible mold to a greater or lesser
degree along the width of that wall. A single swab sample was collected from the
dark material on the back of the exposed wall.
The closet area was moisture tested and found to be dry but extensive mold was
seen along the lower exposed sheetrock and along and behind the framing at the
back of the closet. Laboratory analysis of the sample indicated extremely elevated
levels of Stachybotrys among other molds at lower levels. Care should be taken
while cleaning.
Recommendations: A qualified mold remediation should be retained to do this
following the guidelines elaborated in IICRC S 520 (mold remediation standards,)
clean and dry the area that has been impacted by mold.
1. Use respiratory protection and protective clothing to insure worker safety;
2. Contain the area to be cleaned to prevent spores from entering the non-
impacted areas and provide negative pressure to the extent possible;
3. Remove insulation and bag while inside the contained area;
4. Flood cut(24") both the closet and the wall across the hallway to expose
possible damaged areas. If visible mold is encountered at the 24"line,cut up
to 48"as needed.
5. HEPA vacuum all accessible areas in the designated work area to remove
loose spores;
6. Clean the area with an EPA approved cleaner. Mold has followed the path of
the moisture and is visible behind framing on both the closet wall and the
wall adjacent to the bathroom. Cleaning these areas may require some
removal of parts of the framing or additional sheetrock.

Re: Digital Diagnostic Laboratories, LLC Project Number 09-10-2014RP1
The attached report is provided by Digital Diagnostic Laboratories, LLC (DDL) from the samples
submitted 09/10/2014 08:21 PM. The results presented in this report are based on the samples
submitted to DDL and represent the conditions present at the time of collection and no claim is made to
associate these analyses with any specific event. It is important to note that the presence or absence of
mold is in part due to environmental conditions. Employees of DDL are not involved in the collection of
samples and therefore can not provide information regarding details of the collection site.
Digital Diagnostic Laboratories, LLC employs only qualified individuals to perform microscopic analysis
of samples. All individuals have at least a bachelor's degree in Microbiology, Mycology, the Biological
Sciences, or sufficient experience in the field of mold identification. In addition all have undergone
rigorous training in the specifics of microscopic mold identification.
DIS-1 Air Sampler
The DIS-1 air sampler is a calibrated device that actively pumps air at exactly 15 liters per minute. As
the air is pulled through the air sampler, it passes a specially coated microscope slide where airborne
particles are deposited. These slides are then used to take photomicrographs using the Digital DIS-10
microscope system. The photomicrographs are then electronically submitted to DDL with a chain of
custody/inspection report where they are analyzed. Results are reported in mold spores per cubic
meter of air.
Swab Samples
In the field swab samples are collected using all-in-one plastic tube containing diluent for preparation of
microbial and environmental sampling (bulb end) and sterile swab (tube end). They are then rehydrated
(by breaking the bulb seal) and the Digital DIS-10 microscope system is used to create
photomicrographs from the sample. The photomicrographs are then electronically submitted to DDL with
a chain of custody/ inspection report where they are analyzed. Results are reported as no mold
detected, 1+ (1-50 spores), 2+ (51-150 spores), 3+ (151-600 spores), and 4+ (greater than 600 spores.)

General Comment:
Molds are everywhere in the air we breathe and are found indoors and outdoors. Numerous molds do
not pose a health risk however, some molds produce chemicals called mycotoxins that can cause flu-
like symptoms or other health concerns. Eliminating your exposure to molds can control and/or assist
mold related health issues.
For toxic mold contact an abatement specialist. If you have health symptoms contact your
doctor/allergist.
Analysis completed on Thur 09/11/2014 02:30 AM
Note: Quantities of mold swabs inspected are graded from none detected to 4+,air samples are spores per cubic meter of air, which denote the
highest numbers observed under direct microscopic examination for examination and analysis.
Background Particulate Density:This rating indicates the presence of airborne particulates other than mold(pollen,dander,insects,Oct.).As the
Background Particulate Density increases, the ability to visually detect smaller mold spores(such as Penicilium/Aspergillus or Acremonium)
decreases. The Level of Debris can be interpreted using the following scale:
Low-Very little particulate present. Virtually no spores undetectable.
Low-Medium-Little particulate present. 97%likelihood that all spores have been counted.
Medium-Moderate level of particulate present. 95%likelihood that all spores have been counted.
Medium-High-increased level of particulate present. 75%likelihood that all spores have been counted.
High-Very heavy particulate.Less than a 75%likelihood that all spores have been counted.
Digital Diagnostic Laboratories does not associate these analyses with any event or significance other than the organisms were present in the
submitted samples. The interpretation of this report should not rule out the presence or absence of other organisms.
