Check the appropriate project type in Box A. Friable asbestos includes popcorn ceiling material, sheet vinyl flooring,cement asbestos board siding,and duct insulation. Nonfriable asbestos is normally found in vinyl floor tiles,window putty and most roofing materials. Step 2. Enter property owner information in Box B. Step 3. Enter the asbestos contractor or property owner information, if the property owner is conducting a single-family residential project,in Box C. Print clearly this is your return mailing label. Step 4. Enter the site address for all notifications in Box D. For multi-structure projects, attach supplemental sheet with a site map (include an address for each site)and a list of the type and amount of friable asbestos to be removed from each structure. Step 5. Check either asbestos survey or material presumed in Box E. All demolitions require that an Asbestos Hazard Emergency Response Act(AHERA)asbestos survey be conducted by a certified AHERA building Inspector. Attach a copy of the survey to the notification of a demolition project when only nonfriable asbestos or no asbestos is identified on the survey. Step 6. Enter the project information in Box F.-arid check the training fire or ordered demolition box if appropriate (a copy of the official order must be attached). All asbestos must be removed prior to conducting a training fire. Additional training fire requirements are contained in Regulation I, Section 8.08. If any nonfriable asbestos materials will be left in place during demolition, check yes and list the type and quantity of material. Step 7. Enter asbestos project information in Box G. (NAI) inspected the storage building located at 35518 661 Avenue SW, in Federal Way for asbestos containing building materials. The structure, built in 1983, has been vacant for several years. It is currently slated for demolition. NAI was accompanied on our inspection by Mr. Jeff J. Wolf, of the City of Federal Way. No other individuals were present and the building was no longer in use during the inspection. While on site, NAI created a basic sketch of the structure for use in depicting bulk sample locations. Building Description The storage shed is a wood-framed structure, resting on wood supports. It encloses approximately four hundred square feet. Exterior walls are clad in wood siding. All eaves and soffitts are also made of wood. The building's two windows are vinyl-framed and do not contain glazing putty. The roof is covered by a layer of building felt, topped by asphalt composite shingles. Inside, the building's walls are made out of gypsum wallboard. In the main room, the walls are not finished, while in the side room, the walls have a light, knock-down texture. The ceilings are also made of gypsum wallboard. Throughout the building, the subfloors are made of wood. In the side room, the floor is covered by a layer of black and white patterned sheet vinyl. The main room floor is bare, but covered with a layer of tan mastic from a previous floor covering. The storaoe building is not heated as does....in_y_12.1 not contain any ductwork. Asbestos containing materials are defined as those building materials containing one percent or more of asbestos as verified by laboratory analysis. Asbestos containing materials are sub-divided into three types: surfacing materials, thermal system insulation, and miscellaneous materials. Surfacing materials are defined as those materials that are sprayed-on, troweled-on or otherwise applied to surfaces including, but not limited to, acoustical plaster on ceilings, paints, fireproofing materials on structural members, or other materials on surfaces for decorative.purposes-` Thermal system insulation means material applied to pipes, fittings, boilers, tanks, ducts, or other structural components to prevent heat loss or gain. All other materials are considered to be miscellaneous materials. All surfacing materials, thermal system insulation and flooring installed in structures prior to 1980 are assumed to contain asbestos unless proven otherwise by a licensed building inspector. Surfacing Materials NAI did not discover any suspect surfacing materials during this inspection. Thermal System Insulation • Fiberglass insulation can be found in all of the building's exterior walls and the ceiling. The insulation is backed with paper, forming batts. Laboratory analysis determined that this insulation does not contain asbestos. Miscellaneous Materials ,; . Asphalt composite shingles cover the building's roof as well as the roof over the entry way. The building's roof contains black shingles, while the entryway is covered by both the black shingles as well as a top layer of brown composite shingles. Building felt covers only the main building's roof. Laboratory analysis determined that neither the black nor the brown shingles contained asbestos. The building felt was also found not to contain asbestos. • Black and white sheet vinyl covers the floor in the building's side room. In total, NAI estimates there to be 60 square feet of the sheet vinyl in the room. Beige mastic adheres the sheet vinyl to the wood subfloor. The floor in the building's main room is covered with a layer of mastic apparently identical to the mastic under the existing sheet vinyl. In total, there appears to be roughly 400 square feet of mastic within the building. Laboratory analysis determined that neither the sheet vinyl nor the associated mastic contains asbestos. _. • Gypsum wallboard covers all of the buildings walls. In the side room, the gypsum wallboard is finished with a fight, knockdown texture, while the main room's walls are unfinished. The seams between each gypsum panel are sealed using joint compound. Laboratory analysis determined that neither the wallboard nor the joint compound contains asbestos. PSCAA regulations require that all asbestos containing materials be properly handled during demolition and renovation and that a survey of the asbestos containing materials be provided to all contractors and posted at the site to prevent potential accidental asbestos exposure. All asbestos must be removed prior to demolition or renovations in which it will be disturbed. Contractors are advised that the potential to find asbestos-containing materials including thermal system insulation in hidden spaces is always present during demolition or renovation projects. Unless notified in writing to return samples, Carolina Environmental, Inc. will discard all bulk samples after 30 days. Many vinyl floor tiles have been manufactured using greater than 1%asbestos. Often the asbestos was milled to a fiber size below the detection limit of polarized light microscopy. Therefore, a 'None Detected" (ND)reading on vinyl floor tile does not necessarily exclude the presence of asbestos. Transmission electron microscopy provides a more conclusive form of analysis for vinyl floor tiles. It is certified by the signature below that Carolina Environmental, Inc. is accredited by the National Voluntary Accreditation Program (NVLAP) for the analysis of asbestos in bulk materials. The accredited test method is EPA/600/M4-82/020 for the analysis of asbestos in building materials. Procedures described in EPA/600/R-93/116 have been incorporated where applicable. Check the appropriate project type in Box A. Friable asbestos includes popcorn ceiling material,sheet vinyl flooring,cement asbestos board siding,and duct insulation. Nonfriable asbestos is normally found in vinyl floor tiles,window putty and most roofing materials. Step 2. Enter property owner information in Box B. Step 3. Enter the asbestos contractor or property owner information, if the property owner is conducting a single-family residential project,in Box C. Print clearly this is your return mailing label. Step 4. Enter the site address for all notifications in Box D. For multi-structure projects, attach supplemental sheet with a site map (include an address for each site)and a list of the type and amount of friable asbestos to be removed from each structure. Step 5. Check either asbestos survey or material presumed in Box E. All demolitions require that an Asbestos Hazard Emergency Response Act(AHERA) asbestos survey be conducted by a certified AHERA building Inspector. Attach a copy of the survey to the notification of a demolition project when only nonfriable asbestos or no asbestos is identified on the survey. Step 6. Enter the project information in Box F. and check the training fire or ordered demolition box if appropriate (a copy of the official order must be attached). All asbestos must be removed prior to conducting a training fire. Additional training fire requirements are contained in Regulation I, Section 8.08. If any nonfriable asbestos materials will be left in place during demolition, check yes and list the type aid quantity of material. Step 7. Enter asbestos project information in Box G. List types of friable asbestos material to be removed: surfacing material such as popcorn ceilings or plaster,sheet vinyl flooring,duct and pipe insulation,cement asbestos board siding or pipe,etc. Step 8. For owner-occupied Single-Family Residential projects, check BOX H1A for renovation projects or BOX H1B for demolition projects (with or without asbestos removal). Asbestos removal may be conducted after a complete notification is received, but demolition activities can only begin on the 10th day after the notification is received. Note: If the single family residence is owned by one family who has been or will be using the residence as their domicile,boxes IA or 1B may be checked A single family residence does not include rental property,multi family units,or any mixed-use building. For Commercial asbestos projects(or projects that do not qualify as Single Family Residential);check the project category H2- 5 that matches the amount of friable asbestos that will be removed. If a demolition is involved, include the appropriate surcharge (additional fee) in your payment. To file for an emergency asbestos or demolition project, check the appropriate box 1 —5 and the applicable emergency box in H6. All emergency requests must be accompanied by a letter from the property owner demonstrating the need to conduct the project immediately in accordance with the requirements in Regulation III,Section 4.03 (c). Step 9. Please certify the accuracy and completeness of the information provided by signing the notification in Box I. Mandatory amendments to the notification are required for changes that increase the project category, change the types of asbestos materials to be removed and changes to start date, completion date and work schedule for asbestos projects. No fee is required for work schedule changes if the contractor is participating in the Agency work schedule fax program. A $25.00 processing fee is required for all amendments. . (NAI) inspected the storage building located at 35518 6th Avenue SW, in Federal Way for asbestos containing building materials. The structure, built in 1983, has been vacant for several years. It is currently slated for demolition. NAI was accompanied on our inspection by Mr. Jeff J. Wolf, of the City of Federal Way. No other individuals were present and the building was no longer in use during the inspection. While on site, NAI created a basic sketch of the structure for use in depicting bulk sample locations. Building Description The storage shed is a wood-framed structure, resting on wood supports. It encloses approximately four hundred square feet. Exterior walls are dad in wood siding. All eaves and soffitts are also made of wood. The building's two windows are vinyl-framed and do not contain glazing putty. The roof is covered by a layer of building felt, topped by asphalt composite shingles. Inside, the building's walls are made out of gypsum wallboard. In the main room, the walls are not finished, while in the side room, the walls have a light, knock-down texture. The ceilings are also made of gypsum wallboard. Throughout the building, the subfloors are made of wood. In the side room, the floor is covered by a layer of black and white patterned sheet vinyl. The main room floor is bare, but covered with a layer of tan mastic from a previous floor covering. T stora e building is-not heated and does not contain any ductwork.
