CONDITIONS:
1. In the event that any ground-disturbing activities or other project activities related to this development
uncovers protected cultural material (e.g., bones,shell, antler, horn,or stone tools) an Inadvertent
Archaeological and Historic Resources Discovery Plan must be implemented as provided by DAHP.
2. GeoResources must be on site during construction to monitor slope stability conditions and confirm
construction activities do not adversely affect site stability.
3. A letter, prepared by GeoResources,verifying construction of the wall is in compliance with
recommendations of the geotechnical report must be submitted to the City no-later than 30 days after the
walls have been finalled.

4.An as-built survey of the retaining wall is required to be submitted to the City after the wall is
constructed. This will provide baseline data for monitoring any long-term movement of the wall.
5. Gravel must be placed at the end of the perf pipe to protect ground surface behind bulkhead.

This letter summarizes our final site observations and construction monitoring observations
of the slope remediation/stabilization, including drainage improvements at the site. We
previously provided a geotechnical report for the site that provided recommendations and
design criteria for stabilization of a landslide that extended onto your property, effecting the bluff
area immediately west of your residence. A structural design was provided for the
recommended repair through the piling/wall contractor, Pile King (McDowell NW).
We provided construction monitoring of the pile installation and wall construction on an
intermittent basis, including final performance testing of select H-piling within the wall
configuration. The piling were driven in general accordance with our recommendations and the
structural engineering design. Treated timber lagging were then installed between the piling to
construct a structural retaining wall system, also in accordance with our recommendations and
the structural design criteria.
Also in accordance with the approved plans, a tightline was installed to collect the drainage
from the northwest downspout drainage and direct it to a constructed erosion pad above the
existing concrete block bulkhead. The tightline was anchored to the slope area using steel
rebar driven into the native soils, mechanically affixed to the tightline.
Based on the above, it is our opinion that the constructed retaining wall and drainage
systems were installed in general accordance with our recommendations and the approved
project plans. It is further our opinion that these improvements have improved the stability of
the area above/behind the retaining wall tinder the existing conditions, and should help protect
the residential structure at the site.
