(EPI) is pleased to present the results of an asbestos survey conducted at the Former Bank Building (subject structure) located at 33370 Pacific Highway South Federal Way, Washington (subject property) on August 9, 2016. The general location of the subject property is indicated on Figure 1. It is EPI's understanding that this survey was requested in anticipation of demolition of the subject structure. The general objective of this survey was to identify asbestos -containing materials (AGMs) used during construction and remodeling of the subject structure. This survey was performed in general accordance with Puget Sound Clean Air Agency (PSCAA) Regulation III Article 4 using sampling protocols in Asbestos Hazard Emergency Response Act (AHERA; 40 CFR 763.86). The sections that follow present information concerning the purpose of the investigation, the methodology EPI used to conduct the survey and collect samples, the results of the investigation, and our conclusions and recommendations. The process of identifying ACMs involves using existing knowledge combined with visual observations and collecting bulk materials samples where appropriate. Existing knowledge includes available information contained in construction documentation or previous surveys. A visual inspection is conducted to confirm the presence of such materials. Using these sources of information, an inspector can develop an inventory of suspected ACMs. To positively identify the asbestos content of the suspect material, a representative number of samples are submitted to a laboratory for analysis. 1.1 Asbestos Survey Objectives The objectives of this asbestos survey are as follows: • Identify construction materials in the structures surveyed that are suspected of containing asbestos; • Sample suspect materials to positively identify ACMs by polarized light microscopy (PLM) analysis of representative samples; • Estimate the quantity of ACMs determined to be present for each material identified,, where possible The survey documented herein was conducted based upon readily observable conditions and materials at the subject structure. If additional potential or suspected ACMs not documented in this report become apparent during demolition, those materials should be tested for the presence of asbestos and handled according to applicable regulations. For usage Conditions Agreement see www.bxwa.com - Always Verify Scal 1 0 • Asbestos Survey Former Bank Building 33370 Pacific Highway South, Federal Way„ WA September 9,2016 2.0 ASBESTOS SURVEY AND SAMPLING METHODOLOGY The first part of this asbestos survey included a visual inspection of all accessible areas of the subject structure to identify "homogeneous materials" that are suspected to be ACMs. For purposes of the asbestos survey, "homogeneous materials" are defined as materials that have uniform physical properties (e.g., color, texture, etc.) and are used throughout the subject structure. ACMs are generally classified into three categories: surfacing materials, which include spray -on or troweled -on materials; thermal system insulation (TSI), which includes insulation and materials associated with HVAC systems; and miscellaneous materials, which include ceiling and floor tiles, roofing materials, and all other materials that do not fall under the other two categories. 2.1 Sampling Guidelines EPI utilizes inspectors who have received AHERA training and certification in Building Inspection and followed AHERA sampling protocols (i.e., 40 CFR 763.86) for this project. If a building owner wishes to establish that a suspect material is non -asbestos, they must submit a minimum number of samples for analysis, and all results must be negative for asbestos content AHERA specifies the number of samples an inspector should take for different sizes and types of suspect materials. For most materials, a single sample is sufficient for sampling purposes. Multiple samples must be taken from: (1) surfacing materials where there is a possibility for differences in mixtures (e.g., plaster); or (2) areas of excessive size or materials with excessive linear footage (e.g., large segments of pipe insulation). Homogeneous materials with these characteristics may require multiple samples to account for the possibility of component variability within a defined area or linear footage. 2.2 Sample Collection Standard sampling tools and safety precautions were used to obtain samples from the suspect materials. The samples were bagged and submitted to a laboratory for analysis. The locations of the representative bulk samples were noted using a coordinate system within the building. Samples were collected in locations that were readily accessible or areas that permitted access to underlying materials. Therefore, it is possible that there may be hidden ACMs in the subject structure that could not be observed during this survey. If discovered in the future, such materials should be sampled prior to removal or disturbance, or assumed to be an ACM and handled and managed according to applicable regulations. 2.3 Laboratory Analysis The PLM method was used to analyze samples collected during this survey. PLM is semi -quantitative and cannot reliably quantify less than about 1 percent asbestos content. This limit corresponds to the lower limit of asbestos content in the AHERA definition of `asbestos -containing material." For the purposes of this survey, a material shall be considered asbestos -containing if, upon laboratory analysis, it is found to contain greater than 1 percent asbestos. For usage Conditions Agreement see www.bxwa.com - Always Verify Scal 2 Asbestos Survey Fonner Bank Building 33370 Pacific Highway South, Federal Way, WA September 9, 2016 EPI submitted 68 bulk samples collected from the subject structure to NVL Laboratories Inc., 4708 Aurora Avenue North, Seattle, Washington 98103. 3.0 ASBESTOS INSPECTION AND LABORATORY RESULTS 3.1 Building Details Bank Building (Former Bank Building — 33370 Pacific Highway South) The subject structure is one Former Bank Building constructed by Seafirst bank in 1980. A general representation of the subject facility is presented on Figure 2. Based on EPI's observations, the subject structure was generally built using standard masonry construction (i.e., brick and mortar) with several interior wood framed walls. The floors of the subject structure are concrete construction. The floors in the interior and restroom spaces were covered with various types of carpet, floor tiles, vinyl floor sheeting and ceramic tiles. The ceilings of the office and restroom spaces were either constructed of drop ceiling tiles or gypsum wallboard. The roof of the subject structure is constructed of layered, tar -covered asphaltic roofing material. 3.2 Suspect Materials A visual inspection of the structures surveyed identified a total of 9 suspected AGMs. Each of these materials is identified by a unique Suspect Material ID in Table 1 and displayed on Figures 3 through 5. In general, the materials included; • Asphaltic roofing material; • Tar papers; • Duct sealant with silver paint • Multiple different walls constructed of painted gypsum wall board; • Multiple types of cove base with mastic; • Sink coating; • Multiple types of drop ceiling tiles; • Carpet with mastic on floors and walls; • Multiple types of putty; • Ceramic wall and floor tiles, grout, and mastic; • Multiple types of vinyl floor sheeting and mastic; • Multiple types of vinyl countertops and mastic; • HVAC baffles; and • Multiple types of floor tiles with mastic. As mentioned above, there may be other ACMs present in the subject structure that could not be inventoried and/or sampled during this survey due to a lack of destructive sampling and/or health and safety concerns. Any other materials encountered during demolition activities that are not described in Table 1, should be sampled prior to removal or disturbance, or assumed to be an ACM and handled and managed according to current applicable regulations. For usage Conditions Agreement see www.bxwa.com - Always Verify Scal Asbestos Survey Former Bank Building 33370 Pacific Highway South, Federal Way, WA September 9, 2016 3.3 Asbestos: -Containing Materials Identified The EPA classifies ACMs as those materials that contain 1 percent or more of asbestos by volume. As previously indicated in Section 3.2, a total of 68 materials were identified as potential ACMs. EPI personnel sampled and submitted 68 of the 68 materials for analysis. Laboratory analytical results indicate that 5 of the 68 materials sampled are confirmed ACMs. A comprehensive list of confirmed AGMs and estimated quantities are included in Table 1. ACMs are also identified on Figures 3 through 5. The original laboratory reports are included as Attachment A. Laboratory analytical results indicate that the following materials located in the subject structure are confirmed ACM (see Figures 3 through 5): Roofing tar paper (Suspect Material ID No. EX -A) — approximately 850 square feet, located on the roof of the drive thru overhang. • Silver paint (Suspect Material ID No. EX -E) — approximately 75 square feet, located on the duct work of the HVAC system on the roof. • Black asphaltic mastic (Suspect Material ID No.. B1 -B) — approximately 100 square feet, located in the former video surveillance/server room in the south west corner of the building. 12" Gray tile and black/brown asphaltic mastic (Suspect Material ID No_ B1 -J) — approximately 900 square feet located in the southern region of the building (bathrooms, hallway, offices, and kitchen). • 12" Black the surrounding gray tile and black/brown asphaltic mastic (Suspect Material ID No. B1 -K) — approximately 300 square feet located in the southern region of the building (bathrooms, hallway, offices, and kitchen). All of the materials listed above must be properly removed and disposed prior to planned demolition or renovation activities. 3.4 Friability EPI did not observe confirmed ACM during this survey that was friable to manual pressure. However, one confirmed ACM (i.e., EX A — roofing tar paper) was fibrous in nature. In its current location, material EX -A is not friable, however, during renovation and/or demolition, the fibrous paper may become friable. Prior to demolition or renovation, it may be necessary to make additional evaluations of ACM friability based upon the forces that will, or may, act upon an ACM during the demolition or renovation. Prior to demolition or renovation, these materials should be inspected for and sampled for asbestos content, or assumed to be ACM and disposed of accordingly. 4.0 FINDINGS AND RECOMMENDATIONS EPI conducted an asbestos survey of the subject structure located on the subject property. The findings of this survey include the following; EPI confirmed the presence of five ACMs at the subject property as discussed above. These materials must be handled and managed according to current applicable regulations prior to any planned redevelopment activities. • One of the confirmed ACMs (i.e., EX -A — roofing tar paper) in its current location is not friable. However, during renovation and/or demolition, the fibrous material may become friable. * Clue to health and safety factors, and threats to current operation of the building, EPI was unable to inspect for gaskets, seals, or paper liners on mechanical equipment, pipes, and behind incandescent light fixtures and electrical panels throughout the building structures. Prior to redevelopment, it would be prudent to inspect and sample these materials for asbestos content if present. 5.0 DISCLAMER As applicable and available within the project schedule and budget, we have completed the agreed scope of services, employing professional standards applicable in the industry today. It should be acknowledged that some potential ACMs may have been obstructed from view or inaccessible at the time of this survey, Such materials may only become apparent during demolition or remodeling activities. We assume no risk for existing conditions on the site. To the extent that these services have required judgment, there can be no assurance that fully definitive or desired results were obtained or, if any results were obtained, that they are supportive of any given course of action. EPI's services have included the application of judgment to scientific principles and to that extent, certain results of this work have been based on subjective interpretation. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light. microscopy (PLM) with dispersion staining in accordance with both EPA 600/11144-82-020, Interim Method for the Determination of Asbestos in Bulk Insulation Samples and EPA 600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by calibrated visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. The detection limit for the calibrated visual estimation is <1 %, 400 point counts is 0.25% and 1000 point counts is 0.1 % Samples are archived for two weeks following analysis. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with both EPA 6001M4-82-020, Interim Method for the Determination of Asbestos in Bulk Insulation Samples and EPA 600/R-931116 Method for the Determination of Asbestos in Bulk Building Materials. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer),. The asbestos concentration in the sample is determined by calibrated visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. The detection limit for the calibrated visual estimation is <1 %, 400 point counts is 0.25% and 1000 point counts is 0.1 % Samples are archived for two weeks following analysis.
