Dear Mr. Sproul
This letter will serve as our notice of completion of the asbestos abatement and debris clean up
for the above mentioned address per proposal 4 2014-7562. Northwest Abatement Services, Inc.
perfornied the following:
Northwest Abatement Services, Inc. removed and disposed of select asbestos containing
materials found in the proposed demolition area. Materials included vinyl floor coverings,
underlayment and select roofing materials. Asbestos containing materials were removed using
certified personnel and according to all the state and local codes. All asbestos containing
materials were removed during this abatement project. This project focused on removing select
materials that would allow the debris clean-up of the residential site. The only structure to remain
at the above listed residence was the 2 car garage.
