This letter provides information requested in the Correction Notice prepared during the
Building Inspection of October 2, 2015.
It is in reference to Item 8 - Request for Engineer's Adjustment Design and Inspection of
Crawlspace Access (copy attached).
On November 14, 2015, at the request of the owner, this office visited the above site and
looked at a number of items noted on the Correction Notice.
It was found that the floor access to the crawlspace located in the closet space was
substantially the correct size, however a floor joist remained blocking the opening. To
remove the interfering joist section, a structural cross blocking to the adjacent joists was
required. This office prepared a detailed shop drawing showing the correct method of
constructing the cross blocking and the support of the interrupted joist. Attached is a
photograph of the access as found and a copy of the shop drawing.
On April 23, 2016 the site was visited and the work removing the blocking joist section was
complete. The work was found to have matched the shop drawing requirements very well.
Attached are photographs of the completed opening.
The measured opening was found to be 17" x 28". This is slightly narrower than the
standard 18" but was compensated for by being longer than the standard 24". Personally
using the access indicated it is a good and comfortable opening and substantially meets
the intent of the standard size requirements.
No remedial work is required for the crawlspace access opening
assistance and consideration in this worlra_m m _

This letter provides information requested in the Correction Notice prepared during the
Building Inspection of October 2, 2015.
It is in reference to Items 6 and 7 - Request for Engineer's Inspection of Foundation
Anchor Bolts and Discussion of Moisture Protection of TJI's (copy attached).
On November 14, 2015, at the request of the owner, this office visited the above site and
looked at a number of items noted on the Correction Notice. Verified during the visit, and
with documentation photographs from the owner, the use of conventional wet placed J -
bolts was verified. The bolts were 1/2" x 10", with about 6.5" embedment into the
stemwall. Spacing was about 4'. These bolts are per the approved plans.
The only deviation from current practice noted was the use of the standard round
washers supplied with the bolt when purchased. These have since been replaced with
the 3" square washers. Sill plates were pressure treated, and verified with the owner as
having membrane moisture barrier on their bottom side.
The second item of concern involved protection of the ends of the TJl joists from
moisture. The construction of the floor system for this addition used top hooked joist
hangers passing down the inside of the stemwall. As a result, the ends of the TJI joists
are against the inside of the concrete wall. Since the structure is substantially complete,
little can be done about the concern for protecting the joist ends.
Generally we would require some fairly drastic revision work to provide a positive
moisture separation for the ends of the joists. Upon a more thoughtful consideration, and
applicable only for this particular situation, we have determined that what is in place is
acceptable. This is only true because of several combined factors specific to this case.
First, the ends of the joists do not press hard against the concrete wall, and in fact
typically have a gap of 1/16th to 1/4 of an inch. Secondly, the crawlspace is very open
and has good cross ventilation. Thirdly, the portion of the stemwall where the joists butt
is typically one to two feet above outside grade. In other words, ground moisture is not a
factor here.
Lastly, the existing house grade is slightly higher in grade than where the addition is
located, and the ground around the addition slopes distinctly down away from the
crawlspace in all three directions. The result is a naturally dry crawlspace.
Again, and for this situation only, no remedial work is required for the moisture protection
of the ends of the TJI joists.
In summary, and based on the observations during the visit, no additional alterations or
remedial measures are required for the anchor bolts or for joist moisture protection.
Attached are photographs of the installed J -bolts. The larger square washers are now
installed instead of the small round ones shown.

This letter provides information requested in the Correction Notice prepared during the
Building Inspection of October 2, 2015.
It is in reference to Item 9 - Comment reference Non -Pressure Treated Wood Form
Material Left in Place (copy attached).
On several visits to the site we did not observe the source for the above inspector's
comment. Consultation with the owner did not reveal where the wood form material had
been left in place. Ongoing photographs by the owner showing later work around the
completed foundation did not reveal old formwork material left in place.
Without observing the wood left in place, we may comment that if plywood or dimension
wood components were left in place following the foundation concrete placement, and then
were backfilled against, there should be no problem.
Provided the wood material did not touch new wood construction and that the concrete
foundation was cast properly, the old form wood left in place has no significance. If it has
no remaining purpose and is in non-structural foundation wall backfill, it may remain in
place and decompose naturally without harm to the structure. No identified areas of the
building addition require or show the use of wood in contact with the ground for any
structural or design purpose.
In summary, unless structurally important wood was left in place and in contact with the
ground, this item of concern may be ignored.

This letter provides information requested in the Correction Notice prepared during the
Building Inspection of October 2, 2015.
It is in reference to Item 1 - Request for Engineer's Inspection of concrete foundation
(copy attached).
On November 14, 2015, at the request of the owner, this office visited the above site
and inspected the in-place and poured concrete foundations for the addition. Based on
field measurements, the dimensions of the stemwall and footing were per the approved
plans. The concrete work looked good, with few if any rock pockets, overall meeting
good placement standards.
While the reinforcing bars were covered, the owner furnished us with a number of good
photographs of the bars in place. The photographs were sufficient to verify good steel
placement and size of members.
Attached are photographs made during our visit and by the owner prior to concrete
pour.
Based on the observations during our visit and the supplied photographs, the foundation
was constructed properly and per plan. No alterations or remedial measures are
required.

This letter provides information requested in the Correction Notice prepared during the
Building Inspection of October 2, 2015.
It is in reference to Items 3 and 4 - Request for Engineer's Inspection of shear wall and
roof construction (copy attached).
On January 3, 2016, at the request of the owner, this office visited the above site and
inspected the three constructed shear walls and roof sheathing for the addition. At the
time of the inspection, the exterior wall siding and roofing were in place, covering the
outside surfaces. The interiors were open framing and accessible. The shear wall and
roof sheathing was 1/2" plywood in all areas.
Per approved plans, the shear walls (all three sections) were designated P1-6,
indicating an 8d nailing at 6" edge and 12" field. The nail size was verified by noting the
few nails which had missed the studs. They were 8d.
The spacing in all wall and roof areas were found to be typically 6" or closer.
Measurements of spacing were determined by forcing out the sheathing from the inside
a tiny amount a running a very thin knife blade along the stud. (The forced out areas
were kept localized and the distance out was about 1/32" to 1/16".)
The following measured nail spacings were found —
North Wall 4"- 2.5"- 4"- 3.25" East Wall 4.25"- 2"- 2.5% 4.5% 6"
South Wall 2.25"- 4.5"- 6.5% 7% 2.5% 4.25"
Roof (South side) Edge 4% 7% 3"- 3.5"
Roof (South side) Field 1 "- 4% 3.5"- 4n- 10% 13"
Tyvek membrane was noted in very limited minor areas around the exterior, but was
sufficient to verify its installation. The exterior siding and composition roofing were
observed and appeared very nicely and properly installed.
Attached are photographs made during our visit and showing the nailing measurement
work.
Based on the observations during our visit, the wall and roof sheathing and nailing, and
their exterior siding/roofing, were all properly constructed and per plan. No alterations
or remedial measures are required.

Per the contractor the existing foundation has 5/8" Dia. anchor bolts spaced at 48" o.c. at
all walls. Therefore retrofit anchor bolts will be required at locations where shear wall types
"SW2" and °SW3" occur and the anchor bolts were not installed per the spacing indicated on
the shearwall schedule.
Where retrofit anchor bolts are required add additional 1/2" Dia. Simpson Strong Bolt 2
Wedge Anchors (or equal) in-between the existing anchor bolts that are spaced at 48" o.c.
so that bolt spacing will be less than the spacing originally shown on the plan. Retrofit anchor
bolts are to have 2-3/4" minimum embedment into the concrete.
For example: For a "SW2" with the existing bolts spaced at 48" o.c, add an additional retrofit
anchor in-between the existing bolts such that the spacing between bolts will be 24" o.c.
