On behalf of New Cingular Wireless PCS, LLC ("AT&T") we are pleased to submit this request to modify AT&T's
existing wireless communication site at the location referenced above, as an Eligible Facilities Request for a minor
modification under Section 6409.
Scope of Work
AT&T proposes the following minor modifications to this site: The removal of three (3) panel antennas and the
installation of six (6) panel antennas, six (6) remote radios, one (1) distribution unit and relocating existing
amplifiers as further described in the construction drawings. (Please note: all work will be performed wholly
within the existing premises and utility easements; this site is free of any concealment features; and the project
otherwise complies with the site's original conditions of approval.)
