I, , state as follows:
I have made application for a building permit from the City of Federal Way, Washington.
2. I understand that state law requires that all building construction contractors be registered
with the State of Washington. The exceptions to this requirement are stated under Section
18.27.090 of the Revised Code of Washington (RCW), a copy of which is printed on the
reverse side of this affidavit.
3. I understand that prior to issuance of a building permit for work which is to be done by any
contractor, the City of Federal Way must verify either that the contractor is registered by the
State of Washington, or that one of the exemptions stated under RCW 18.27.090 applies.
4. In order to provide verification to the City of Federal Way of my compliance with this
requirement, I hereby attest that after reading the exemptions from the registration
requirement of RCW 18.27.090, I consider the work authorized under this building permit to
be exempt under No. � and will therefore not be performed by a registered contractor.
I understand that I may be waiving certain rights that I might otherwise have under state law in
any decision to engage an unregistered contractor to perform construction work.

1) No building shall encroach onto any building setback line or
easement shown or not shown.
2) Maximum building height is 30 feet above the average building
elevation as per Federal Way City Ordinance #90-51.
3) Maximum driveway width is 20 feet.
4) The Driveway shall be paved per section 22-1453 of the Federal
Way City Code. The driveway shall be paved from the existing
roadway pavement edge, or curb, to the garage or carport.
5) Building setbacks are: 20 feet front; 5 feet side; 5 feet rear.
6) Detached garage on any residential property is allowed only as an
accessory structure. Per FWCC Sec. 22-946, accessory uses,
facilities and activities are permitted only if they are
associated with the primary use permitted on the property. Also
FWCC Sec. 22-946 requires that the accessory use, facility or
acitivity must be clearly secondary to the permitted use.
7) Service connections for electrical and communication facilities
shall be placed underground per section 16-48 of the Federal Way
City Code.
8) Required downspout footing drains, and run-off from impervious
surfaces shall be tightlined to an approved storm drain system as
required per the approved drawings/plans. Call for inspection of
system prior to backfilling trenches. Final building inspection
will not be conducted until storm system is approved.
9) Prior to any clearing or grading on a lot, the owner/builder
shall install temporary erosion/sedimentation control facilities
approved by the City. These facilities must insure that dirt or
sediment laden water does not enter the public drainage system,
adjacent lots or public streets. The owner/builder bears the
responsibility to maintain the facilities in proper working order,
replacing as necessary. The facilities may be removed only after
such time as construction is complete and landscaping is
installed. See attached for standards and site plan for location
of silt fencing.
10) The contractor must obtain a ROW permit to construct the new
driveway approach and abandon the existing driveway approach.
