The survey was performed by Jason Carlson on January 28, 2020; a copy of his Asbestos Hazard Emergency Response Act (AHERA) building inspector certification is attached. The purpose of the assessment was to assist Helix Design Group Inc, with communicating the presence, location and quantity of asbestos–containing materials to employees and contractors working in the Project Area and to meet the requirements for an asbestos survey for the Puget Sound Clean Air Agency (PSCAA) and a good faith inspection as required by Washington State Department of Labor and Industries’ Division of Occupational Safety and Health (DOSH) regulations prior to renovation. As required by Washington Administrative Code (WAC) 296-62-077 and PSCAA Regulation III, Article 4, an Asbestos Hazard Emergency Response Act (AHERA) accredited building inspecter performed the survey. A copy of the building inspector certificate is attached to this letter report. ASBESTOS SURVEY The roof consisted of metal panels with fiberglass batt insulation, multiple sealants, caulking, and mastics. Helix Design Group, INC. (SAT). SAT is accredited through the National Voluntary Laboratory Accreditation Program (NVLAP) of the U. S. Department of Commerce. This accreditation does not constitute endorsement, but rather a finding of laboratory competence (certification copy is attached). SUMMARY/CONCLUSION There were no asbestos-containing materials identified during the roof survey. Any concealed building materials discovered during work activities, which are suspected to contain asbestos, should be sampled by an AHERA-accredited asbestos building inspector and analyzed by a NVLAP-accredited laboratory to confirm the presence of asbestos prior to the disturbing such materials. WAC 296-62-07721 and PSCAA Regulation III, Article 4 requires that this survey report to be placed on-site during renovation and/or demolition and copies provided to the contractor(s) bidding and performing work.
