Previous environmental review included a Drainage Technical Information Report, geotechnical report, traffic report and concurrency reviews. Previous SEPA DNSs issued April25, 2001 and March 13, 2010. 9. Do you know whether applications are pending for governmental approvals of other proposals directly affecting the property covered by your proposal? If yes, explain. A number of buildings have been permitted, but construction schedule may be deferred beyond the current land use approval (08-10555-00-UP). Give brief, complete description of your proposal, including the proposed uses and the size of the project and site. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. (Lead agencies may modify this form to include additional specific information on project description.) The project involves construction of the remaining 174 independent or assisted living apartments, 40 units of memory care housing, and 17 independent living cottages. The project is adjacent to the existing Village Green of Federal Way seniors housing community, which includes 167 independent and assisted living units. In lieu of several Cottages, a community building may be constructed depending on the activity level and abilities of the campus residents as the project is developed over time. The proposal is for a 5-year land use approval with a second 5-year extension (total of 10 years) for phased construction. With this proposal, the total number of units on the entire Village Green Campus would be 398 units. This is 8% more than previous reviews and approvals, which were at 367 units. This change is primarily the result of deleting some of the cottages and adding the memory care building. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. The site is located between 1st Avenue Sand 2nd Avenue SW, north of SW 356th Street. The site extends northwesterly off the end of 2nd Avenue SW. The site includes King County Tax Parcels 302104-9017 and 302104-9146, all located in the NE of Section 30, Township 21 N, Range 4 E, W.M. B. ENVIRONMENTAL ELEMENTS 1. Earth a. General description of the site: (circle one): Flat railing, hilly, steep slopes, mountainous, other Parcel 9017 (Buildings F—J) has an existing paved lane and a gravel parking area. The remainder is landscaped or wooded with flat to rolling topography. Parcel 9146 (Memory Care and Cottages) has been previously developed into "ready to build" sites with roads and utilities. Some infrastructure modification is required for the Memory Care building. Topography is rolling. b. What is the steepest slope on the site (approximate percent slope)? There are steep slopes (>40%) present near the southwest corner of the site. These slopes were identified and reviewed through previous land use approvals and the initial ddevelopment and site improvements. No work or disturbance is proposed on or near these slopes. Slopes within the development areas are on the order of 2 to 10%, other than where the site was designed to "stair step" down between the roads, with over/under Cottage units. See the slope analysis diagram on the clearing and grading plan. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any agricultural land of long-term commercial significance and whether the proposal results in removing any of these soils. Site soils consist of silt, sand, gravel and cobbles. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. None noted. e. Describe the purpose, type, total area, and approximate quantities and total affected area of any filling, excavation, and grading proposed. Indicate source of fill. Earthwork is expected to involve approximately 9,000 CY of cut and 3,000 CY of fill. Road base and paving materials will be imported from an approved source. f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe. Some short term erosion is likely during construction. After construction the site will be generously landscaped to prevent erosion. g. About what percent of the site will be covered with impervious surfaces after project construction (for example, asphalt or buildings)? Approximately 54%. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: A Construction Stormwater Pollution Prevention Plan will be prepared and implemented as part of future site development activities. Erosion and sedimentation control measures are expected to include: stabilizing construction access; filter fabric fencing; catch basin protection socks; sediment trapping ponds; covering exposed soils; phasing clearing activities; and other maintenance and housekeeping practices. 2. Air a. What types of emissions to the air would result from the proposal during construction, operation, and maintenance when the project is completed? If any, generally describe and give approximate quantities if known. Some dust & exhaust emissions will occur during construction. Minor automobile exhaust will occur after completion including employee, resident and visitor. Traffic (and exhaust) is expected to be less than a typical residential housing development of similar size. b. Are there any off -site sources of emissions or odor that may affect your proposal? If so, generally describe. None. c. Proposed measures to reduce or control emissions or other impacts to air, if any: Sprinkling of exposed soils (during active earthwork) and covering of soils (plastic, mulch and vegetation) can be employed to minimize dust migration during construction. Hylebos Creek Wetland and Hylebos Creek tributary J-11 is located approximately 670 feet northeast of the site. 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. Ural 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. None. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. None. No. 5) Does the proposal lie within a 100-year floodplain? If so, note location on the site plan. 6) Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. None. b. Ground Water: 1) Will groundwater be withdrawn from a well for drinking water or other purposes? If so, give a general description of the well, proposed uses and approximate quantities withdrawn from the well. Will water be discharged to groundwater? Give general description, purpose, and approximate quantities if known. None. 2) Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (for example: domestic sewage; industrial, containing the following chemicals... ; agricultural; etc.). Describe the general size of the system, the number of such systems, the number of houses to be served (if applicable), or the number of animals or humans the system(s) are expected to serve. The western portion of the site utilizes an existing infiltration pond for storm water management. Where will this water flow? Will this water flow into other waters? If so, describe. The western portion of the site utilizes an existing infiltration pond for stormwater management. The eastern portion of the site may implement permeable pavement and infiltration for some portion of stormwoter management. Additional runoff from the eastern portion of the site will be collected by existing storm drains and routed to an existing detention pond. 2) Could waste materials enter ground or surface waters? If so, generally describe. Waste materials typically associated with residential development may enter surface waters. Vra 3) Does the proposal alter or otherwise affect drainage patterns in the vicinity of the site? If so, describe. d. Proposed measures to reduce or control surface, ground, and runoff water, and drainage pattern impacts, if any: Catch basins, sediment storage, wet ponds, bioswales and storm water filtration will all be employed to mitigate storm water quality impacts to current standards. Infiltration and conservation flow control will be provided to mitigate stormwoter quantity impacts. No drainage patters will be altered. Public sewers will collect sanitary sewer. Landscaping will be professionally maintained. 4. Plants a. Approximately 6.1 acres of clearing will occur including native trees and shrubs as well as areas that were previously cleared. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any: Most significant trees within the perimeter landscape buffer will be retained. Many additional significant trees will be retained on the western margin of the property. Landscaping will also re -incorporate a percentage of native trees, shrubs and groundcover. Tree density, per FWRC is achieved. Refer to the Landscape Plans. e. List all noxious weeds and invasive species known to be on or near the site. Some blackberries are present on and near the site. 5. Animals a. List any birds and other animals which have been observed on or near the site or are known to be on or near the site. Examples include: birds: hawk, heron, eagle, songbirds, other: Crow, robin, starling, ducks (in the stormwater ponds), hummingbirds, blue jays and other songbirds are present. mammals: deer, bear, elk, beaver, other: Racoon, possum, squirrel and other small mammals are likely present. fish: bass, salmon, trout, herring, shellfish, other: None. b. List any invasive animal species known to be on or near the site. None known. 6. Energy and Natural Resources a. What kinds of energy (electric, natural gas, oil, wood stove, solar) will be used to meet the completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. Electricity and natural gas will be the energy source for the project (heating, cooling and lighting). b. Would your project affect the potential use of solar energy by adjacent properties? If so, generally describe. No. c. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts, if any: Buildings will conform to the Washington State Energy code. Retention of significant trees on the south and wes#side of the development will help reduce cooling needs in the summer by providing shade and may help to reduce heat loss in the winter by providing some shelter from prevailing winds. 7. Environmental Health a. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste, that could occur as a result of this proposal? If so, describe. 1) Describe any known or possible contamination at the site from present or past uses. None. 2) Describe existing hazardous chemicals/conditions that might affect project development and design. This includes underground hazardous liquid and gas transmission pipelines located within the project area and in the vicinity. None. Typical fuel and lubricants in the construction equipment and paints & solvents are the only anticipated toxic or hazardous chemicals expected during construction. Paints and solvents will be used for typical occasional building maintenance. Typical commercial building cleaning chemicals will be used on a regular basis. 4) Describe special emergency services that might be required. No special emergency services are anticipated. 5) Proposed measures to reduce or control environmental health hazards, if any: A Spill Prevention, Control and Countermeasure Plan will be developed in accordance with City of Federal Way and WSDOE guidelines at the time of site development/building construction permitting. b. Noise 1) What types of noise exist in the area which may affect your project (for example: traffic, equipment, operation, other)? Traffic noise from SW 356th Street and 1st Avenue south exists at the site, but does not substantially impact the project. 2) What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis (for example: traffic, construction, operation, other)? Indicate what hours noise would come from the site. Typical construction noise (excavation equipment, truck traffic, sawing, hammering, etc.) will occur during active construction periods. Construction hours are limited by city code, but would typically occur between 7 a. m. to 6 p.m. on weekdayys. Lang term noise from project traffic is expected to be minimal (less than with typical single family development). 3) Proposed measures to reduce or control noise impacts, if any: Landscape buffering will help reduce current and long term noise impacts. Code limitation on work hours will minimize construction noise. 8. Land and Shoreline Use a. What is the current use of the site and adjacent properties? Will the proposal affect current land uses on nearby or adjacent properties? If so, describe. Other than 1, 4-plex cottage building and infrastructure improvements, the site is vacant. The site has current land use approval for Seniors Housing. The property to the north is also seniors housing consisting of the existing Village Green seniors housing campus. Property to the southwest and west is single family. To the south and east are SW 3S6th Street and Ft Avenue S, respectively. How much agricultural or forest land of long-term commercial significance will be converted to other uses as a result of the proposal, if any? If resource lands have not been designated, how many acres in farmland or forest land tax status will be converted to nonfarm or nonforest use? Past farmland usage is unlikely as the site is or was mostly forested before past grading and infrastructure improvements. Forest cover appears to be second or third growth, but the site is not been part of a working forest or subject to any forest management plan. No. 1) Will the proposal affect or be affected by surrounding working farm or forest land normal business operations, such as oversize equipment access, the application of pesticides, tilling, and harvesting? If so, how: c. Describe any structures on the site. The site currently contains one 4-plex senior housing building, as well as infrastructure improvements including: roads, utilities, sidewalks, street lights and landscaping. None. d. No. A well head protection area exists to the north on the existing Village Green campus, but does not extend onto these subject properties. i. Approximately how many people would reside or work in the completed project? Approximately 450 people will reside in the completed buildings, with perhaps 20 employees. j. Approximately how many people would the completed project displace? None. k. Proposed measures to avoid or reduce displacement impacts, if any: Project provides additional housing. Proposed measures to ensure the proposal is compatible with existing and projected land uses and plans, if any: Seniors housing is allowed in the RS 7.2 and RS 9.6 zone through the Process 4 land use review. The site has been approved for seniors housing since 2000, through previous Process 4 reviews and development has gradually proceeded as market (demand and construction) conditions hove deemed appropriate. So the project is compatible with existing land uses. Surrounding properties are developed at or near full potential, so projected land uses are expected to remain the some into the foreseeable future. in. Proposed measures to reduce or control impacts to agricultural and forest lands of long-term commercial significance, if any: Not applicable. 9. Housing a. Approximately how many units would be provided, if any? Indicate whether high, middle, or low- income housing. Approximately 219 market rate an 12 affordable housing units will be provided. b. Approximately how many units, if any, would be eliminated? Indicate whether high-, middle-, or low-income housing. None. c. Proposed measures to reduce or control housing impacts, if any: Project provides housing. A covenant is recorded to provide 5% affordable units within the project. 10. Aesthetics a. What is the tallest height of any proposed structure(s), not including antennas; what is the principal exterior building material(s) proposed? Structure height as defined by FWRC is expected to be 30feet. Internal ramps to basement garages have been excluded from the low side measurement. Principal exterior materials are expected to include architectural vinyl, cement board, wood, with stone or brick trim. See conceptual architectural plans.. b. What views in the immediate vicinity would be altered or obstructed? View obstructions are not anticipated. Views from surrounding properties will be altered from partially developed to developed and landscaped property. c. Proposed measures to reduce or control aesthetic impacts, if any: Tree retention, landscaping and quality building design will be used to reduce aesthetic impacts. 11. Light and Glare a. What type of light or glare will the proposal produce? What time of day would it mainly occur? Minor residential and pedestrian lighting will occur in the evening hours. Could light or glare from the finished project be a safety hazard or interfere with views? Lighting is not expected to interfere with any views or cause a safety hazard. c. What existing off -site sources of light or glare may affect your proposal? Street lighting is present along the street frontages but is not expected to affect the project. d. Proposed measures to reduce or control light and glare impacts, if any: Not applicable. Lighting will be minor in scale and not directed toward surrounding properties. See lighting plan. 12. Recreation a. What designated and informal recreational opportunities are in the immediate vicinity? The nearest parks include West Hylebos Wetlands Park and Hylebos Blueberry Farm, which are about' mile from the Site. Me b. Would the proposed project displace any existing recreational uses? If so, describe. c. Proposed measures to reduce or control impacts on recreation, including recreation opportunities to be provided by the project or applicant, if any: The site will have open spaces/informal recreation areas; lawn areas for strolling; benches provide outdoor sitting areas for visiting; a path around the perimeter of the campus provides opportunities for quite walks or exercise; pea patches may provide opportunities for gardening. Other opportunities available an the existing Village Green campus include game rooms, inside exercise rooms, a pool & spa, theater, arts and craft space, meeting space for history talks, candidate forums and the like outside areas for small concerts, barbeques and wine%heese events. 13. Historic and Cultural Preservation a. Are there any buildings, structures, or sites, located on or near the site that are over 45 years old listed in or eligible for listing in national, state, or local preservation registers ? If so, specifically describe. No. b. Are there any landmarks, features, or other evidence of Indian or historic use or occupation? This may include human burials or old cemeteries. Are there any material evidence, artifacts, or areas of cultural importance on or near the site? Please list any professional studies conducted at the site to identify such resources. Examples include consultation with tribes and the department of archeology and historic preservation, archaeological surveys, historic maps, GIS data, etc. The Washington Information System for Architectural and Archaeological Records Data was consulted and no information was found concerning cultural or historic resources. Previous SEPA review and public notices to tribes and other state and local entities did not result in response concerning archeological significance. d. Proposed measures to avoid, minimize, or compensate for loss, changes to, and disturbance to resources. Please include plans for the above and any permits that may be required. Not applicable. 14. Transportation a. Identify public streets and highways serving the site or affected geographic area and describe proposed access to the existing street system. Show on site plans, if any. The site fronts 1st Avenue S, SW 356th Street and 2nd Avenue SW. The site will be served from existing access points onto 1st Avenue S and 2nd Avenue Sw. b. Is the site or affected geographic area currently served by public transit? If so, generally describe. If not, what is the approximate distance to the nearest transit stop? Yes. Public transit stops exist directly in front of the Site on SW 356th Street and across the street on 1st Avenue S and for both north -south and east -west connectivity. c. How many additional parking spaces would the completed projector non -project proposal have? How many would the project or proposal eliminate? The project will add approximately 246 parking spaces. None will be eliminated. d. Will the proposal require any new or improvements to existing roads, streets, pedestrian, bicycle or state transportation facilities, not including driveways? If so, generally describe (indicate whether public or private). None anticipated. The frontages (and both main corridors in general) are currently developed with through lanes, turn lanes, bike lanes and sidewalks. A waiver of street improvements was previously approved with the previous land use review and is again requested. M e. Will the projector proposal use (or occur in the immediate vicinity of) water, rail, or air transportation? If so, generally describe. f. How many vehicular trips per day would be generated by the completed projector proposal? If known, indicate when peak volumes would occur and what percentage of the volume would be trucks (such as commercial and nonpassenger vehicles). What data or transportation models were used to make these estimates? As a Continuing Care Retirement Community, the project is estimated to generate 545 total and 36 P.M. Peak Hour trips. Only several truck trips are expected each day (food delivery, courier, waste/recycle). A concurrency application is included with the Process 4 land use application. Proposed measures to reduce or control transportation impacts, if any: The project will provide the required parking on site. Transportation impact fees will also be paid to offset impacts. 15. Public Services a. Would the project result in an increased need for public services (for example: fire protection, police protection, public transit, health care, schools, other)? If so, generally describe. The project will need public services such as fire/life safety, police protection, health care and 911; however, no more so than if residents were living in single amity homes. b. Proposed measures to reduce or control direct impacts on public services, if any. Increase property tax revenues will mitigate impacts. 16. Utilities a.
